Perivesical unicentric Castleman disease initially suspected to be metastatic prostate cancer

Abstract: Unicentric Castleman disease (UCD) is a relatively rare lymphoproliferative disease, which commonly presents as a mediastinal mass and less frequently involves abdomen, pelvis, and retroperitoneum. We report a case of a 64-year-old man with newly diagnosed low-volume, Gleason 3 + 3 = 6 prostate adenocarcinoma, who in considering active surveillance versus treatment was found to have a left perivesical and iliac chain lymphadenopathy concerning for potential metastatic involvement. He underwent magnetic resonan… Show more

“…MRI findings generally show a mass that is relatively isointense to the skeletal muscle as hypointense on T1W and hyperintense on T2W and contrast enhancement after gadolinium; flow voids showed feeding vessels, but these characteristics are non-specific [5 , 6 , 8 , 11] . MRI is suitable for evaluating the extent of the disease and its relationship with adjacent structures, although with limited evaluation of calcifications.…”

“…Unicentric CD responded well to resection, which is considered as the standard treatment. The removal of unicentric CD is feasible in the majority of cases, and it is almost always curative [4] , even for cases arising from the pelvic lymph nodes; nearly 100% with 5 years survival [11] . However, recurrences have been reported, especially for incomplete resection [4] .…”

“…There is less vascular proliferation in comparison with hypervascular pattern [38]. Recently, two subtypes were described: HHV-8 associated type (especially in HIV-patients), and the multicentric not otherwise specified (NOS) type [39,40]. In the HHV-8 type, the lymph node architecture is generally normal and either follicle hyalinization or marked interfollicular vasculature can be noticed.…”

“…After surgical removal, the clinical symptoms will disappear and the seric level of IL-6 decreases [48]. If the complete removal cannot be achieved, the alternative is represented by radiation therapy [40]. It is difficult to treat and completely eradicate the multicentric disease.…”
