Periprosthetic seroma with false-positive FDG PET-CT reactive nodes mistaken for metastases in a patient previously treated of metastasic melanoma. Potential source of diagnostic errors

Abstract: HighlightsProsthetic meshes used for repair abdominal defects may lead to false-positives FDG PET/CT uptake.Awareness of false positive FDG PET/CT secondary to periprosthetic seroma is essential.A positive diagnosis is needed to avoid misinterpretations and inappropriate treatments.

“…An identification of potential false-positive results on PET/CT is vital to ensure an accurate interpretation during oncologic image reporting by taking into account the clinical context because many inflammatory and reactive phenomena are known to cause false-positive avidity (9). In our case, FDG-avid LNs at axillary and supraclavicular areas were initially interpreted as distant metastatic LNs.…”

“…Benign lymphadenopathy in the vaccine injection site nearest regions is considered a very common consequence, not only after COVID vaccination; it is also reported as an effect of H1N1 and human papillomaviruses vaccines [ 6 , 7 ]. Hypermetabolic lymphadenopathy is usually identified as an increased lymph nodes uptake on PET-CT occurring not only in cancer-related situations but also during an occurring inflammatory process [8] .…”

“…A mild degree of FDG uptake can be noted in seroma. [ 6 7 ] Spontaneous degradation of keratin and cellular debris, sudden blockage of lymphatic flow with lymph fluid filling into the rest of the potential spaces can lead to the formation of cystic lymph node metastases in cases of SCC. [ 8 ] A cystic lymph node can be FDG avid depending upon the tumoral content in the periphery.…”
