Periprocedural Polypharmacy in Lumbar Fusions Performed Under Spinal Anesthesia Compared With General Anesthesia

Abstract: BACKGROUND:Spinal anesthesia (SA) is a safe and effective alternative to general endotracheal anesthesia (GEA) for lumbar surgery. Foremost among the reasons to avoid GEA is the desire to minimize postoperative cognitive dysfunction (POCD). Although POCD is a complex and multifactorial entity, the risk of its development has been associated with anesthetic modality and perioperative polypharmacy, among others.OBJECTIVE:To determine whether SA reduced polypharmacy compared with GEA in patients undergoing transf… Show more

“…
Postoperative cognitive dysfunction (POCD), as a neurological complication that occurs several days after surgery, is manifested as anxiety, insanity, as well as impairments of personality and learning and cognitive abilities. 1,2 Its mechanism is rather complicated. According to current findings, central inflammatory response can disrupt neurocytes to cause the occurrence of POCD, which resembles the case of degenerative diseases.

“…27 Furthermore, compared with GEA, SA is associated with more favorable hemodynamic stability during surgery, reduced risk of myocardial infarction, lower risk of hypoxic episodes in the postanesthetic care unit, lower pain scores, less postoperative nausea and vomiting in the postanesthetic care unit, and decreased perioperative polypharmacy. 28,29 However, there remains a great need to investigate the aforementioned patient outcomes and symptoms in the context of high-dose SA.…”

“…13 The benefits of awake or non-GA spine surgery have also been identified in the context of postoperative urinary retention, where a study by Hernandez et al 18 determined this type of anesthesia to be superior when compared with GA, even after adjusting for confounders. Finally, a study by Olmos et al 19 also pinpointed other advantages for a decrease in perioperative polypharmacy, cutting the average number of postoperative medications administered by half and significantly decreasing the need for intraoperative vasopressors.…”
