Peripheral Neuropathies in Systemic Lupus Erythematosus: Clinical Features, Disease Associations, and Immunologic Characteristics Evaluated Over a Twenty‐Five–Year Study Period

Abstract: Objective. To characterize peripheral neuropathy subtypes, ancillary studies, and immunologic profiles associated with peripheral neuropathies in patients with systemic lupus erythematosus (SLE).Methods. In this 25-year study of 2,097 SLE patients, we characterized peripheral neuropathies due to SLE and compared clinical and SLE-related features in patients with versus those without neuropathy.Results. The prevalence of peripheral neuropathies was 5.9% (123 of 2,097 patients), and 66.7% of these patients (82 o… Show more

“…The majority manifest as peripheral neuropathy [69], which includes mono or poly-neuropathy, cranial-neuropathy, inflammatory demyelinating poly-radiculoneuropathy, and plexopathy. A recent finding is that 17% of SLE-related peripheral neuropathies are small-fiber neuropathy [70]. Small-fiber neuropathies can cause severe burning pain by targeting unmyelinated C fibers and thinly myelinated A fibers.…”

“…[1] Disease manifestations range from fatigue, skin rash, and arthralgias to central nervous system (CNS) involvement, arthritis, serositis, nephritis, pneumonitis, cardiac disease, and hematological problems. [2,3] Due to its chronic nature, unpredictable course and widespread potential for harm, patients with SLE have a shorter life expectancy and reduced quality of life (QoL) compared to healthy sedentary population. [4] Sexuality has been cited as an important part of the whole person, and sexual expression has been described as a crucial part of personal's self-identity.…”

“…Patients may present with sudden weakness requiring aggressive immunosuppressive therapy. Although small-fiber neuropathy is not included in the American College of Rheumatology definitions, Oomatia reported a prevalence of 17.1% in SLE patients with peripheral involvement 5,49 . Electrodiagnostic studies are usually normal and a skin biopsy demonstrates loss of intraepidermal nerve fibers.…”
