Peripheral nerve ultrasound in amyotrophic lateral sclerosis phenotypes

Abstract: Ulnar nerve ultrasound in ALS subgroups revealed significant differences in distal CSA values, which suggests it has value as a marker of LMN involvement. Its potential was particularly evident in the UMND and PLS groups, which can be hard to separate clinically, yet their accurate separation has major prognostic implications.

“…The nerve cross‐sectional area (CSA) was calculated by continuous manual tracing of the nerve circumference, excluding the hyperechoic epineural rim. Peripheral nerve ultrasound is well established locally with very good to excellent intra‐ and interrater agreements …”

“…to have possible, probable, or definite ALS according to the revised El Escorial criteria . Of those 55 cases, 29 (53%) and 13 (24%) of patients, respectively, were included in our previous cross‐sectional and longitudinal peripheral nerve sonography ALS studies . Four subjects (7%) met the criteria for comorbid FTD .…”

“…Intuitively, one may expect reduced CSA in axonal PN due to loss of myelinated fibres. However, this is seldom apparent with the exception of modest reduction of nerve calibre in amyotrophic lateral sclerosis (ALS) 60 65 66. In fact, US of axonal PN may detect nerve enlargement in approximately 20% of patients 15…”

“…Schreiber and colleagues reported thinning of ulnar nerves in different subtypes of ALS, primarily based on the involvement of the upper and lower motor neurons (4). Because evaluation of cervical roots might provide information in the proximal muscles innervated by C5, C6, and C7 nerve roots, sonographic evaluation of peripheral nerve (e.g., median and ulnar nerves) as well as cervical nerve roots would be useful (3).…”
