Background and aim: Neutrophil-lymphocyte ratio (NLR) and lymphocyte-monocyte ratio (LMR) are associated with clinical outcomes in malignancy, cardiovascular disease and stroke. Here we investigate their association with outcome after acute ischaemic stroke treated by mechanical thrombectomy (MT). Methods: Patients were selected using audit data for MT for acute anterior circulation ischaemic stroke at a UK centre from May 2016-July 2017. Clinical and laboratory data including neutrophil, lymphocyte and monocyte count tested before and 24 h after MT were collected. Poor functional outcome was defined as modified Rankin Scale (mRS) of 3-6 at 3 months. Multivariable logistic regression analyses were performed to explore the relationship of NLR and LMR with functional outcome. Results: One hundred twenty-one patients (mean age 66.4 ± 16.7, 52% female) were included. Higher NLR (adjusted OR 0.022, 95% CI, 0.009-0.34, p = 0.001) and lower LMR (adjusted OR − 0.093, 95% CI (− 0.175)−(− 0.012), p = 0.025) at 24-h post-MT were significantly associated with poorer functional outcome when controlling for age, baseline NIHSS score, infarct size, presence of good collateral supply, recanalisation and symptomatic intracranial haemorrhage on multivariate logistic regression. Admission NLR or LMR were not significant predictors of mRS at 3 months. The optimal cut-off values of NLR and LMR at 24-h post-MT that best discriminated poor outcome were 5.5 (80% sensitivity and 60% specificity) and 2.0 (80% sensitivity and 50% specificity), respectively on receiver operating characteristic curve analysis.Conclusion: NLR and LMR tested at 24 h after ictus or intervention may predict 3-month functional outcome.

Objective: To evaluate the role of inflammatory markers for distinguishing malignant and benign ovarian masses.Methods: Preoperative demographic, clinicopathologic, and laboratory variables were reviewed in patients with an ovarian mass that was subsequently diagnosed as either epithelial ovarian cancer (EOC) or a benign ovarian mass on histologic analysis. The differences between variables of the two groups were further evaluated. Logistic regression analysis was applied to evaluate variables to predict the presence of EOC.Results: According to the analysis of 229 patients with EOC, 120 (52.4%) patients had serous adenocarcinoma. Of the 229 patients, 110 (48.1%) patients had stage I or II disease and 119 (52.0%) had stage III or IV disease. There was a significant difference between EOC and benign ovarian mass in median values of variables such as age, white blood cell (WBC) count, hemoglobin concentration, platelet count, cancer antigen 125 (CA125) levels, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and lymphocyte-to-monocyte ratio (LMR) (all P < 0.001, except for WBC count [P = 0.009]). In addition, there was significant difference in median values of these continuous variables among early-stage EOC, advanced-stage EOC, and benign ovarian mass (P < 0.001 for all variables). On multivariate logistic regression analysis, age (odds ratio [OR] = 4.14, P < 0.001), CA125 levels (OR = 9.87, P < 0.001), NLR (OR = 1.76, P = 0.049), PLR (OR = 2.41, P = 0.004), and LMR (OR = 0.51, P = 0.024) were found to significantly predict the presence of EOC.Conclusion: The three LMR, NLR, and PLR markers were found to be predictors for the presence of EOC. Further prospective studies to assess these markers as screening tools for the presence of EOC are required.

The lymphocyte / monocyte ratio (LMR) obtained by dividing the number of lymphocytes by monocyte number is considered a new marker of inflammation. Recent studies have found an inverse relationship between LMR levels and 3-month functional recovery after acute ischemic stroke (AIS). Our aim in this study is to determine the role of LMR in the short-term mortality of AIS patients. Method: Our study is a single centered retrospective study. 508 patients with AIS, who presented to our clinic between January 2011 and December 2017 and 512 healthy control with similar age and sex were included to our study. Results: While monocyte count was higher in the patient group; lymphocyte count and LMR levels were higher in the control group. The patient group was then divided into two subgroups according to 30-day mortality. When the patients in the first group survivied (n = 396), the patients in the second group died within 30 days after AIS(n = 112). In the second group, the lymphocyte count and LMR levels were lower when the monocyte count was elevated (p < 0.001). Logistic regression analysis showed that uric acid, CRP and LMR are independent variables for the occurrence of 30-day mortality. In addition, ROC analysis revealed that an LMR value below 2.95 can be used as a marker for the short-term mortality of AIS (AUC:0.77, %95 CI:0.67-0.86,sensitivity:%72.6, specificity: %80.7). Conclusions: Based on our results, it can be concluded that lower LMO levels in patients with AIS are related to shortterm mortality.
