Introducción: La miocardiopatía periparto es una afección potencialmente mortal asociada al embarazo, que se presenta en los últimos meses de gestación e incluso hasta después del parto. Su presentación clínica, abordaje diagnóstico y manejo suelen ser semejantes a los de otras cardiomiopatías. Caso clínico: Mujer multigesta de 29 años, con antecedente de hipertensión gestacional, cursando su quinto día de puerperio presenta un cuadro de falla cardiaca acompañado de edema agudo pulmonar. Se describen el abordaje diagnóstico y el manejo brindando en un centro de tercer nivel del norte de México, incluyendo ventilación mecánica no invasiva, y los buenos resultados que se obtuvieron con el empleo de múltiples terapias farmacológicas y no farmacológicas. Conclusiones: La miocardiopatía periparto sigue siendo una gran desconocida, por lo que su sospecha por los médicos que reciben a estas pacientes es muy reducida. Afortunadamente, reconocer a una paciente con falla cardiaca e instaurar un manejo de acuerdo con guías internacionales brinda muy buenos resultados.

Despite remarkable improvements in the past two decades, the annual cardiovascular mortality rate has remained higher for women than for men. Pregnant women represent an underinvestigated population in clinical research, and the mechanisms of long-term cardiovascular complications in women with obstetric complications remain to be elucidated. Regarding advanced heart failure during pregnancy, interventional approaches are effective but still underutilised. Percutaneous mechanical circulatory support is a valuable option for peripartum cardiogenic shock, although its use during pregnancy is still limited. Survival rates have improved in recent years, but further emphasis on the importance of early recognition and initiation of heart failure treatment in this patient group is warranted. The aims of this review are to summarise the current literature on the implementation of mechanical circulatory support in cardiogenic shock during pregnancy and delivery and to understand the role of percutaneous ventricular assist devices in the management of such conditions.

Peripartum cardiomyopathy (PPCM) is an uncommon disorder of the cardiovascular system and is linked to high rates of morbidity and mortality. It is an idiopathic condition characterized by left ventricular systolic dysfunction with an ejection fraction of approximately 45% near the end of pregnancy or immediately after delivery. Anesthesia management in these women is challenging due to low physiological reserve and potential negative effects on the fetus. To ensure that mother and child are supported safely through delivery, careful anesthesia control is required. Here, in this review article, we discuss the anesthetic implications in preoperative, operative, and postoperative phases in women with perioperative cardiomyopathy undergoing vaginal delivery or cesarean section.
