Our meta-analysis found no significant difference in the choroidal thickness both under the fovea and around the optic nerve head between OAG patients and controls. On the basis of the anatomic features of blood supply in optic nerve head, it is plausible that the choroidal thickness is not an appropriate parameter to evaluate the damage of OAG, and choroidal thinning may not be an important component of glaucomatous optic neuropathy.

ZusammenfassungHintergrund Vaskuläre Theorien zur Pathogenese des primären Offenwinkelglaukoms (POWG) und zur papillären Blutversorgung lassen Veränderungen der peripapillären Aderhaut bei POWG vermuten. Die prospektive Studie vergleicht die peripapilläre Aderhautdicke (pAD) bei POWG mit Augengesunden.Material und Methoden Die pAD wurde mittels SD-OCT in 300, 600 und 900 μm Abstand zur Papille für 12 Halbmeridiane bestimmt. An der Studie nahmen 15 Probanden mit POWG (kaukasisch, Alter 65,7 ± 10,6 Jahre) und 9 augengesunde Probanden (kaukasisch, Alter 59,4 ± 14,7 Jahre) teil.Resultate In Augen mit POWG war die pAD um durchschnittlich 15,2 % (89,0 ± 34,4 versus 105,4 ± 26,8 μm, p < 0,001) reduziert. Der Unterschied zum Normalauge wird mit zunehmendem Abstand zur Papille geringer: 300 µm: 20,5 % (77,9 ± 33,7 versus 98,0 ± 25,0 μm, p = 0,002), 600 µm: 12,9 % (93,2 ± 32,6 versus 107,0 ± 27,3 μm, p = 0,002), 900 µm: 12,7 % (97,2 ± 32,4 versus 111,3 ± 26,4 μm, p = 0,002). In Bezug auf die Halbmeridiane waren in den verschiedenen Papillenabständen im nasalen Bereich signifikante Unterschiede der pAD nachweisbar. Nach Bonferroni-Holm-Korrektur ist keine Signifikanz der pAD-Unterschiede mehr vorhanden (p > 0,05 für alle Halbmeridiane).Schlussfolgerung Bei Patienten mit POWG ist die pAD signifikant reduziert. Das Ergebnis unterstützt die vaskuläre Theorie für die Pathogene des POWG. We appreciate the opportunity to respond to Aksoy and colleagues, 1 whom we thank for their interest in our article. 2 Their comments focused on factors that influence the thickness of the choroid, which we agree with. The interest in investigating the association between choroidal thickness and glaucoma has increased recently as a result of improvement in imaging deep ocular structures. The number of publications on this topic has increased following the introduction of enhanced depth imaging technique with spectral domain optical coherence tomography. Although findings are conflicting with regard to open-angle glaucoma, a careful review of the literature indicates that the weight of evidence favors the lack of association between the two. Prior to the article discussed herein, our group also reported such a finding after comparing choroidal thickness in patients with glaucoma and nonglaucomatous controls.3 Two other things that transpired in that study were: (1) there is a large amount of variability in choroidal thickness among healthy subjects; and (2) even after controlling for identifiable factors, the thickness of the choroid may still be under the influence of unknown factors that may differ among individuals. These observations laid the foundation for the study on choroidal thickness in unilateral advanced openangle glaucoma. In this correspondence, we would like to provide further clarification in response to two specific points raised by Aksoy et al.
