Periorbital Aging and Ethnic Considerations: A Focus on the Lateral Canthal Complex

Abstract: In comparing young and aged cohorts, African American women demonstrate a more dramatic attenuation of the lateral canthal complex than their Caucasian counterparts. It appears that the lateral canthal complex has been underappreciated, and it is a vital component to periorbital rejuvenation.

“…Since the appearance of the face is one of the most important measures for the evaluation of facial ageing, many studies have attempted to use qualitative measures to evaluate the progression of facial ageing and the response to rejuvenation treatments via the use of facial photographs as a primary assessment tool. 3,4,[20][21][22] Few radiology studies have been used for the assessment of facial ageing. CT has been used to evaluate age-related changes in deeper facial structures such as facial bones.…”

“…Periorbital agerelated changes include crow's feet and lower eyelid rythides, scleral show, infraorbital hollowing, herniated fat pads, excess upper and lower lid skin, festoons, and eyelid hooding. In addition, attenuation of the lateral canthal tendons results in loss of the youthful architecture of the eye secondary to a decrease of the aesthetically pleasing upward tilt [7,15,17,18,[22][23][24][25][26][27]. The change in the lateral canthal complex position is functionally important, as its changes contribute to lower eyelid laxity, which may result in rounding of the lateral commissure and narrowing of the palpebral fissure [6, 8-12, 14, 28].…”

“…Superficial muscle thinning is induced by chronic NIR exposure, which ultimately leads to superficial tissue aging and skin ptosis [14] [19]. Fair skin tends to wrinkle and sag earlier in life [28] [29], and characteristic age-related skin changes occur at a more accelerated rate in Caucasians [30]. Thus, patients seeking facelifts often have fair skin and/or a habit of sunbathing.…”
