Perioperative Coagulation Management in Liver Transplant Recipients

Abstract: We review contemporary coagulation management for patients undergoing liver transplantation. A better understanding of the complex physiologic changes that occur in patients with end-stage liver disease has resulted in significant advances in anesthetic and coagulation management. A group of internationally recognized experts have critically evaluated current approaches for coagulopathy detection and management. Strategies for blood component and factor replacement have been evaluated and recommended therapies… Show more

“…We simultaneously treat severe consumptive coagulopathy (DIC), or DIC‐like TEG patterns with enhanced fibrinolysis, with both heparin (to inhibit coagulation activation) plus an antifibinolytic (to inhibit fibrinolytic activation; Figure ) . The combination minimizes the risk of catastrophic intravascular thrombosis that is known to occur with antifibrinolytic therapy alone …”

“…Although caution is always warranted, thrombotic risk with these agents is low unless there is an identified preexisting hypercoagulable state. 50,51 Two recent meta-analyses examining the use of antifibrinolytics in GI bleeding and liver transplantation have not demonstrated increased thrombosis risk. 52,53 Antifibrinolytic use has reduced the need for blood products perioperatively in liver transplant, 52 and application of topical forms, in particular, is low risk and may be a useful adjunct for persistent bleeding.…”

“…70,71 Although individual transplant centers typically have algorithm-based guidance for the management of bleeding intra-and postoperatively, general guidelines and transfusion goals are well summarized in a recent review. 50…”

“…Perioperative coagulation management in pediatric liver transplant patients continues to be a moving target. Both end‐stage liver disease and ALF lead to simultaneously hyper‐ and hypo‐coagulable states, resulting in an increased risk for both thromboembolic and bleeding complications . Perioperatively, these severe coagulopathies can trigger frequent transfusions in order to prevent bleeding and pre‐transplant death .…”
