Perioperative chemotherapy with ifosfamide and doxorubicin for high-grade soft tissue sarcomas in the extremities (JCOG0304)

Abstract: Although the toxicities of the regimen were significant, pre-operative chemotherapy followed by post-operative chemotherapy using doxorubicin and high-dose ifosfamide was feasible. The outcome of the trial for the patients with high-grade soft tissue sarcomas in the extremities was favorable, and this regimen is promising for further investigation. This trial was registered at the UMIN Clinical Trials Registry (www.umin.ac.jp/ctr/) as C000000096.

“…The choices of drug combinations and applications are continuously expanding. Recently, Dox has been combined with ifosfamide and tested in a Phase II study for the treatment of soft tissue sarcomas [17]. Albumin-bound PTX particles (Abraxane®) and gemcitabine (Gem) combination were approved by the US FDA as the first-line treatment for patients with metastatic pancreatic cancer [18].…”

“…The most common agents are methotrexate, cyclophosphamide, and bleomycin (2)(3)(4). Ifosfamide is one of the standard drugs for soft tissue sarcomas (5,6). However, interstitial pneumonitis that is clearly associated with only ifosfamide, as in our case, has not been reported, although there have been some studies on interstitial pneumonitis after administration of combined chemotherapy including ifosfamide, which is used for bone and soft tissue sarcoma, and lymphoma (3,(7)(8)(9).…”

“…e results of the study by Tanaka et al support the use of preoperative chemotherapy followed by postoperative chemotherapy consisting of three cycles of doxorubicin 60 mg/m 2 and ifosfamide 10 g/m 2 for localized high-risk extremity or trunk wall STS [13]. In this regard both, the noninferiority of three cycles of a full-dose conventional chemotherapy (epirubicin and ifosfamide) in comparison with five cycles and the association between response to therapy and better rates of overall survival (OS), particularly when Choi criteria are used for the evaluation, have been pinpointed in a randomized trial [11].…”
