Perioperative Care of Adults with Congenital Heart Disease for Non-cardiac Surgery

Abstract: In 2010 it was estimated that 1.2 million young adults in the United States have some form of congenital heart disease (CHD). Data from large adult congenital heart disease (ACHD) centers show that as many as 50 % of these patients are over 40 years of age. As this population ages, physicians are likely to encounter such patients in their practice. Risk stratification and an interdisciplinary approach are paramount in achieving safe outcomes. The assessment of the patient must consider the variable expression … Show more

“…Broadly, patients with simple CHD lesions may undergo noncardiac surgery at a general hospital. Patients with moderate CHD lesions should be assessed at a specialist center prior to undergoing noncardiac surgery at a general hospital, while those with complex CHD should undergo all nonemergency surgeries at a specialized center 29 …”

“…7 A point system for the risk stratification of a CHD patient before undergoing a procedure was developed by Mossad. 8 This classification factors in variables such as complexity and physiology of cardiac lesion, type and urgency of surgery, age, and the duration of preoperative hospital stay. A patient presenting for noncardiac surgery may have undergone a corrective cardiac surgery, may have a residual cardiac lesion after correction, or may have an uncorrected lesion such as a grown-up with CHD (GUCHD).…”

“…A point system for the risk stratification of a CHD patient before undergoing a procedure was developed by Mossad. [ 10 ] Children and adults with heart disease are at increased risk of mortality and morbidity when undergoing non-cardiac surgery. [ 4 9 ] Risk associated with an individual patient is based on several criteria [ Table 2 ].…”
