Periodontal Regenerative Treatment of Intrabony Defects Associated with Palatal Grooves: A Report of Two Cases

Abstract: A palatal radicular groove (PRG) is a morphological deformity, occurring during tooth development. It is usually located on the palatal aspect of maxillary incisors and frequently associated with periodontal or endodontic-periodontal lesions. Some treatment options were described for such lesions, including primary endodontic treatment and periodontal surgery and extraction with intentional replantation after removal of a PRG and endodontic treatment. The present paper reported two cases of PRG-associated deep… Show more

“…Additionally, it was hypothesized that using an enamel matrix derivative could help patients whose inflammatory processes were fast progressing and causing the degeneration of periodontal tissue [ 29 ]. Additionally, severe intrabony flaws linked with the palatal radicular groove were successfully treated without endodontic therapy or retreatment by applying an enamel matrix derivative [ 30 ].…”

“…There are few studies that address detailed morphometry, in millimeters, of this anomaly. Some of this, describe the groove in lateral incisor, on the palatal radicular face [4][5][6]17]. Brin e Ben-Bassat [3] classified the depth of the LCVG in UCI as shallow or deep but did not describe the criteria for such classification and neither quantify in millimeters how this anomaly varied within the sample.…”

“…Recently, cases were reported to handle this condition with innovative strategies including root canal treatment followed by intentional replantation[ 3 , 18 ] and enamel matrix augmented periodontal regeneration[ 19 ], but the long-term follow-ups are necessary to illustrate the effectiveness of these cases. In this case report, under the intentional replantation procedure, the clinician could clean a vast amount of bacterial plaque, granulation tissues, and caries that were often noted on the surface of the grooves.…”
