Periodontal plastic surgery for treatment of localized gingival recessions: a systematic review

Abstract: Overall, PPS was effective in reducing gingival recessions with a concomitant improvement in attachment levels. Even though no single treatment can be considered superior to all the others, CTG was statistically significantly more effective than GTR in recession reduction. Further research is needed to identify the factors most associated with successful outcomes.

“…However, for these systems, the periodontal milieu often poses the major problem that the required period of drug exposure (7-10 days) cannot be achieved [11]. Also in the field of periodontal surgery, as in the transplantation of a mucous membrane [12], resorbability of the scaffold material is important to avoid inflammatory effects and surgical removal. Therefore, the use of films or membranes represents a promising approach in periodontal treatment to promote tissue regeneration by avoiding migration of epithelial cells into the periodontal pocket and incorporating antibiotics to inhibit the related bacteria growth [13].…”

“…In suitable cases a combination of the subepithelial connective tissue graft with a coronally advanced pedicle flap is considered to be the gold standard procedure due to the high success rates reported in the literature. 33,37 …”

“…25 When the subepithelial connective tissue graft is compared with guided tissue regeneration, the subepithelial graft has shown evidence of achieving greater root coverage. 22,33,34 Evidence shows that the size of the initial recession defect will determine the amount of root coverage achieved. 22 Miller class I defects can achieve complete root coverage in 100% of cases and in class II defects complete root coverage was seen in 88% of cases.…”

“…23 Achieving full root coverage following a single periodontal plastic surgical procedure is difficult and success is often considered to be any decrease in amount of exposed root surface (Fig. 1g) ie an increase in gingival height from the mucogingival line to the gingival margin.…”
