Perinuclear anti-neutrophil cytoplasmic antibodies (p-anca) in chronic ulcerative colitis: Experience in a Mexican institution

Abstract: The prevalence of p-ANCA was similar to that reported in other countries. Their presence was associated to UC severity, but offered no more information than the obtained by clinical methods.

“…Although the observed prevalence fits within the previously reported range, it was quite low compared to that observed in Western countries [10,17,21] and is closer to the results of studies initiated in Asian countries [20,22]. Differences in clinical patterns of UC between races or ethnic groups may help us to understand the etiology or pathogenesis of the disease.…”

“…Further studies are necessary to clarify the ethnic In addition to the diagnostic value of pANCA, many studies have evaluated the relationship between pANCA and clinical course or disease activity of UC, but these are still under debate. Several reports have demonstrated a significant relationship between serum titer pANCA and the clinical course or severity of UC [3,6,18,21]. In addition, some investigators have identified a relationship between psychological factors and pANCA status [27,28], while others found no correlation [3,10,25].…”

“…The prevalence of pANCA in Japanese,148 Korean,149 Thai,150 and Romanian151 patients with UC is lower compared with Caucasians, but comparable to Caucasian data in Mexico,152 Estonia,153 and a different Korean cohort 142. pANCA was found more frequently in UC patients with more extensive disease in China147 and Korea,149 with relapse rates higher when pANCA was present in the Korean cohort 149…”

“…This association has been found between ANAs and SLE, 23 and between p-ANCA and UC. 7 We studied the association of ANAs with the activity of UC measured by the Mayo score and no positive correlation was found. In order to establish if ANAs are associated with UC activity we also compared ANAs with CRP levels, which has been demonstrated to be a reliable serological marker of disease activity, 24 -28 and no association was found.…”

“…6 The prevalence of p-ANCA in Mexican UC patients is 51% and seems to be a marker in those with severe activity (P Ͻ 0.0001; odds ratio [OR] ϭ 9; 95% confidence interval [CI] ϭ 3.2-24.7). 7 There is evidence showing that the presence of p-ANCA is also associated with a more aggressive clinical course. 8 On the other hand, Aitola et al 9 reported that p-ANCA titers decrease after proctocolectomy, suggesting that a reduction of the inflammatory process may modify the immunological alteration related to UC.…”
