BackgroundMaternal mental health in the postpartum period is essential for the optimal development of the newborn and appropriate breastfeeding practices. Despite this, a shortage of concrete evidence exists regarding the magnitude of postpartum depression and associated factors. This meta-analysis study was therefore done to fill such a gap.MethodsPubMed, Scopus, and EMBASE were investigated with no time limitation. A manual search for a reference list of articles was also done. Relevant data were extracted using Meta XL package and analysis was done Stata-11 meta-prop package. Heterogeneity was checked with Cochran's Q-statistics and the Higgs I2 test. Furthermore, sub-group and sensitivity analyses were done. Egger's test and funnel plots tests were engaged to identify publication bias.ResultsA total of 16 studies that assessed 11400 postpartum women in Ethiopia were included. The average estimated prevalence of postpartum depression was 21.9% (95% CI: 18.98, 24.77). The pooled prevalence of postpartum depression was higher in studies that used SRQ-20; 24.6% (95% CI: 18.42, 30.84) than studies that used PHQ-9; 18.9% (95% CI: 11.52, 26.28). Moreover, the pooled estimated prevalence of postpartum depression was slightly higher in the southern part of Ethiopia (22.6%) than Addis Ababa (21.2%). Poor marital relation (pooled aOR = 3.56) (95% CI: 2.50, 4.63), unplanned pregnancy (pooled aOR = 3.48) (95% CI: 2.18, 4.79), previous history of depression (pooled aOR = 4.33) (95% CI: 2.26, 6.59), poor social support (pooled aOR = 4.5) (95% CI: 3.34, 5.56), domestic violence (pooled aOR = 3.77) (95% CI: 2.62, 4.92), family history of mental illness (pooled aOR = 4) (95% CI:1.56, 6.56), use of substance (pooled aOR = 4.67) (95% CI:4.00, 5.34), low income (pooled aOR = 2.87) (95% CI: 1.59, 4.14), stressful life event (pooled aOR = 3.5) (95% CI: 1.39, 5.87) and perinatal complications (pooled aOR = 3.8)( 95% CI: 1.45, 6.15) were among the associated factors for postpartum depression in Ethiopia.ConclusionMore than one in five women was with postpartum depression and factors such as poor marital relations, history of depression, poor social support, domestic violence, unplanned pregnancy, family history of mental illness were related to it. Therefore, maternal postnatal care services should be geared to incorporate this public essential health concern.

Background. Antenatal depression is prevalent and serious problems that is associated with psychosocial factors, obstetric history, and history of psychiatric illness. Evidence on prevalence and factors associated with antenatal depression at community level is limited in Ethiopia. The aim of this study was assessing the prevalence of antenatal depression and associated factors among pregnant women in West Badewacho Woreda, Hadiyya Zone, South Ethiopia, 2018. Methods. A community based cross sectional study was conducted from March 15 to April 12, 2018. To draw a total sample size of 541 pregnant women, multistage sampling technique was used. Pretested semi-structured questionnaire and standardized scale was used to collect data from each study subject. Data were entered and cleaned using Epi-Data version 3.1 and exported to SPSS version 23 for analysis. Bivariate analysis was carried out to see crude association between each independent variable and outcome variable. Odds ratios at 95%CI were computed to measure the strength of the association between the outcome and the independent variables. P-value < 0.05 was considered as statistically significant in multivariate analysis. Results. The prevalence of antenatal depression in the study population was 23.3% (CI: 19.8–26.8). Factors significantly associated with antenatal depression were marital status other than married (single, widowed, divorced) [AOR: (2.807; 95%CI: (1.268, 6.227); p-value = 0.042], history of previous depression [AOR: 3.414; 95%CI: (1.154, 12.999); p-value = 0.001] family history of mental illness [AOR: 3.874; 95%CI: (1.653, 7.052); p-value = 0.028], recent violence from intimate partner [AOR: 3.223; 95%CI: (1.359, 7.643); p-value = 0.008], unsatisfactory marital relation [AOR: 7.568; 95%CI: (3.943, 14.523); p-value < 0.001], lack of adequate social support [AOR: 5.491; 95%CI: (2.086, 14.451); p-value < 0.001] and unplanned current pregnancy [AOR: 2.013; 95%CI: (1.025, 3.953); p-value = 0.042]. Conclusion. The prevalence of antenatal depression in west Badewacho woreda was high and it is associated with marital status, unplanned current pregnancy, history of previous depression, family history of mental illness, recent violence from intimate partner, poor marital satisfaction level, and poor social support. Improving maternal and child health services and introducing screening for depression as part of routine antenatal assessment to curb antenatal depression should get due attention.

Prevalence and Predictors of Postpartum Depression: Northwest Ethiopia

Background. Postpartum depression is an umbrella, which encompasses several mood disorders that follow childbirth within 6 weeks. Screening for postpartum depression would improve the ability to recognize these disorders and enhance care that ensures improved clinical outcomes. Early identification of postpartum depression is important in order to plan for implementation strategies that allow for timely treatment and support of women with postpartum depression. Objective. To determine the prevalence and associated factors of postpartum depression among women who gave birth in the last six weeks in Gondar town, Northwest Ethiopia, 2018. Methods. A community based cross-sectional study was conducted among 526 women who gave birth in the last 6 weeks from July 1 to 30, 2018 in Gondar town. Cluster sampling technique was used. Data were collected by semi-structured and pretested questionnaire and entered into epi-Info version 7.0 and then analyzed by SPSS version 20.0. Both bivariate and multivariable logistic regression model were fitted. Adjusted odds ratio with 95% confidence interval has been computed and variables with p-value <0.05 were considered statistically significant. Results. The prevalence of postpartum depression among 526 postnatal women was 25% (95% CI: 21, 28). Abortion history (AOR = 1.79, 95% CI: 1.07, 2.97), birth weight <2.5 kg (AOR = 3.12, 95% CI: 1.78, 5.48), gestational age below 36 weeks (AOR = 2.18, 95% CI: 1.22, 3.88) unplanned pregnancy (AOR = 2.02, 95% CI: 1.24, 3.31), relatives’ mental illness (AOR = 1.20: 1.09–3.05), had no antenatal visit (AOR = 4.05, 95% CI: 1.81, 9.05), had no postnatal visit (AOR = 1.82, 95% CI: 1.11, 3.00) were factors significantly associated with postpartum depression. Conclusion and Recommendations. The prevalence of PPD was found to be higher. Variables like abortion history, low birth weight, gestational age below 36 weeks, unplanned pregnancy, relatives’ mental illness, had no antenatal visit, and had no postnatal visit were predisposing factors to postpartum depression. Preventive measures to avoid low birth weight and pregnancy complications are also identified as proactive ways to reduce postpartum depression. Early identification and treatment of depression during ANC and postpartum care can mitigate the impact of PPD on the mother-baby dyad. Emphasis must be given women to have ANC and PNC follow up.
