Perinatal and Antibiotic Exposures and the Risk of Developing Childhood-Onset Inflammatory Bowel Disease: A Nested Case-Control Study Based on a Population-Based Birth Cohort

Abstract: The role of early-life environmental exposures on Inflammatory Bowel Disease (IBD) onset remains unclear. We aimed to quantify the impact of perinatal conditions and antibiotic use in the first 6 and 12 months of life, on the risk of childhood-onset IBD, in a birth cohort of the region Friuli-Venezia Giulia (Italy). A nested case-control design on a longitudinal cohort of 213,515 newborns was adopted. Conditional binomial regression models were used to estimate Odds Ratios (OR) with 95% confidence intervals (C… Show more

“…The included studies were published between 2010 and 2020 (tables 1A-D): 11 cohort studies 22-32 and 11 case-control studies. “…Given recent evidence suggesting that antibiotic exposure in the first year of life is associated with both IBD [9,40] and asthma [42,43], a sensitivity analysis adjusted for this exposure was performed on 70 IBD cases and 700 references (analyses restricted to children born after 1994). Antibiotic exposure in the first year of life showed a non-significant increased risk of asthma at 3 years (OR 1.62 95% CI 0.91-2.69).…”

“…On the basis of the age at IBD diagnosis, childhood onset IBD was categorized in three groups: pediatric IBD between 10 and 17 years (Paris classification A1b), EO-IBD between 0 and 9 years (Paris classification A1a), and VEO-IBD between 0 and 5 years [8,40].…”

“…This Special Issue of the International Journal of Environmental Research and Public Health (IJERPH) focuses on the current state of knowledge on perinatal and postnatal exposures and adverse pregnancy, maternal, fetal and neonatal outcomes through population-based birth cohort studies, with a specific focus on real-word data. The 12 accepted articles covered a wide range of themes that can be addressed specifically through birth cohort study design; however, only three were based on real word data with record-linkage to HADs [ 10 , 11 , 12 ].…”

“…Two papers focused on inflammatory bowel diseases, both as an outcome of perinatal and antibiotic exposure in early life and as a condition associated with asthma, among children identified in a birth cohort [ 11 , 12 ]. Both exposures and comorbidities were assessed by means of HADs, using a matched case-control design nested in a population-based birth cohort of more than 213,000 individuals born between 1989 and 2012, and resident in the Region of Friuli-Venezia Giulia (Italy).…”
