Pericardial decompression syndrome in a patient with hypothyroidism presenting as massive pericardial effusion: a case report and review of related literature

Abstract: SummaryThe authors present a case of a 44-year-old female with unrecognised hypothyroidism consulting for heart failure symptoms. Echocardiogram revealed massive pericardial effusion with tamponade physiology, attributed to primary hypothyroidism from a previous thyroidectomy. Levothyroxine was started at a dose of 0.7 ug/kg/day followed by subxiphoid pericardiostomy. 9 h postpericardiostomy however, hypotension developed and despite hydration and inotropic support, patient succumbed to cardiogenic shock on th… Show more

“…First, pericardial diseases occur in autoimmune and non-autoimmune hypothyroidism alike: they have been reported in autoimmune thyroiditis,5 10 as well as in hypothyroidism due to iodine deficiency30 and postsurgical hypothyroidism 31. Second, to date, no specific antibodies have been proven to be the culprit of pericardial diseases in hypothyroidism 5 29.…”

“…Patient's hemodynamic status was continuously monitored, during and after the pericardiocentesis. Lim et al have shown association of fatal postprocedural “pericardial decompression syndrome” with pericardiocentesis [ 3 ]. Three liters of hazy, yellowish/“gold paint” pericardial fluid was drained ( Figure 4 ).…”

“…Pericardial effusion and even cardiac tamponade as presenting symptoms of hypothyroidism are rare but have been described earlier [ 12 16 ]. In the general population, the risk of major complications after pericardiocentesis is reported to be about 5% [ 17 ].…”

“…Additionally, it was reported that the cardiovascular system is significantly depressed as the response to catecholamines might be significantly reduced by downregulated expression of adrenergic receptors in hypothyroidism [ 19 ]. Lim et al described a pericardial decompression syndrome, which resulted in severe hemodynamic instability after diagnostic needle pericardiocentesis in a patient with hypothyroidism [ 16 ]. Therefore, the body cannot counteract the hemodynamic effects of a cardiac tamponade with tachycardia and peripheral vasoconstriction [ 16 ].…”
