Perianal Median Raphe Cyst: A Rare Lesion with Unusual Histology and Localization

Abstract: Median raphe cysts present anywhere between the external urethral meatus and the anus. The cysts can occur at parameatus, glans penis, penile shaft, scrotum, or perineum. Perianal region is an extremely rare location for these lesions. Here we present a 50-year-old male patient who presented with a cystic, fluctuant lesion, located at 12 o'clock in perianal region. Microscopic examination revealed a cystic lesion with keratinized and nonkeratinized stratified squamous epithelium, pseudostratified ciliated epit… Show more

“…We would like to extend this classification and add two more categories: ciliated and pigmented. The ciliated type is characterized by the presence of ciliated cells interspersed with pseudostratified [16, 26, 47, 56] or columnar [31, 57, 58] epithelium. The pigmented variant appears brown-black in color due to the presence of melanocytes and melanin pigment in the epithelial lining [10, 46, 50].…”

“…Unal et al [56], in their compilation of cases of ciliated cysts, mentioned six cases, including their own, as documented in the literature. Perhaps they missed the case reported by Navarro et al .…”

“…MRCs can theoretically develop anywhere along the midline from meatus to anus, including parameatus, 9 glans penis, 10,19 penile shaft, 3–5,11,14,15 scrotum, 12 perineum, 7,10 and perianal, 6,16 but they present most commonly in the penile shaft 13 . MRCs commonly appear in childhood or adolescence, with a bimodal distribution at approximately 1–10 and 21–40 years old 13,17,18 .…”

“…MRCs are rare benign lesions of the male genitalia that can develop at any site along the midline from meatus to anus (i.e. parameatus, glans penis, penile shaft, scrotum, perineum, or perianal) 1–19 . The condition was first described by Mermet in 1895, 1 and MRC is currently believed to be caused by a defect in closure of the median raphe during embryonic development 19 .…”
