Performance of Focus ELISA Tests for HSV-1 and HSV-2 Antibodies Among University Students With No History of Genital Herpes

Abstract: In this low-prevalence population, the positive predictive value of the Focus HSV-2 ELISA test was low. This finding, together with those reported elsewhere, indicates that caution is warranted when recommending HSV screening in low-prevalence or heterogeneous populations. Consideration should be given to raising the cutoff index value for defining a positive test result.

“…The differences in specificity results between our data and Hogrefe et al 4 may be related to populations studied as no other explanatory factor has been identified. It is worth noting that although the specificity of the HerpeSelect ELISA has been found to be relatively low among populations with a low HSV-2 seroprevalence (<5%) in the United States,10 or by geographical location (for example, Vietnam11), the specificity seen here in Kisumu is notably lower and may be specific to certain geographical areas of Africa.…”

“…Similarly, studies conducted in South West of Iran showed that the HSV-1 sero-prevalence (79.2%) was higher than HSV-2 (23.3%) (Tayyebi and Sharifi, 2010). In a study conducted in Maryland, Mark et al (2007) obtained an HSV-1 and HSV-2 sero-prevalence of 48 and 3.4% respectively by Western Blot assay. Epidemiologic studies conducted among miners and commercial sex workers in South Africa reported sero-prevalence rates of 60-90% respectively (Auvert et al, 2001).…”

“…There is a debate about the accuracy of the Focus Diagnostics HerpeSelect HSV-2 IgG ELISA test kits used in this study. Some researchers have suggested that the cutoff value for a positive test should be raised from 1.1 to 3.0 or 3.5 when the test is used in low prevalence settings [24,25]. Raising the cutoff value to 3.5 would have affected the study results since 14 of the 32 positive tests fell in the disputed range, decreasing the estimated prevalence from 6.6% to 4.7%.…”
