Performance of diagnostic biomarkers in predicting liver fibrosis among hepatitis C virus-infected Egyptian children

Abstract: The aim of the present study was to identify specific markers that mirror liver fibrosis progression as an alternative to biopsy when biopsy is contraindicated, especially in children. After liver biopsies were performed, serum samples from 30 hepatitis C virus (HCV) paediatric patients (8-14 years)

“…PIIINP levels were higher among chronic HCV patients than in healthy controls in a small adult sample (p=0.0004) and a small pediatric sample (p=0.04) (37). A similar trend was found in another small pediatric HCV population (p<0.01) (48) and in a chronic HBV sample (p<0.05) (60). PIIINP levels correlated with fibrosis stage, and could reliably exclude inflammation and fibrosis in chronic HBV patients.…”

“…Elevated TIMP-1 predicts advanced fibrosis (F≥3) in HIV/HCV co-infected patients (p=0.001) (63). TIMP-1 levels were higher compared to controls in small adult (p<0.0001) and pediatric (p<0.003) chronic HCV samples (37), pediatric HCV (p<0.0001) (48), and in patients with active untreated IgG4-RD (p<0.01) (122). TIMP-1 levels were not different between chronic HCV patients and controls in a small sample (43).…”

“…Loss of hepatic GH-STAT5 signaling causes defects in lipid and glucose metabolism leading to steatosis, hyperglycemia, and insulin resistance. TGF-β 1 is secreted from HSC and Kupffer cells, but with inflammation hepatocytes gradually become the major source of TGF-β 1 [8][9][10][11]; it is the most important cytokine that stimulates the production of extracellular matrix (ECM) by HSC [12] and can induce apoptosis in hepatocytes, leading to cirrhosis [13].…”

“…Aminotransferases are sensitive indicator of hepatocyte injury. The pattern of aminotransferase elevation, increasing AST/ALT ratio can be helpfull to diagnose in patients with chronic hepatitis [6], [7]. The ratio of serum AST/ALT was describe by De Ritis and known as De Ritis ratio.…”
