“…Although CRS can underestimate the true sensitivity (Yu et al . 2020), we achieved a moderately good sensitivity of ~73% and specificity of ~91–94% by I‐PCR based on Ag85 detection against CRS, which may be further improved so as to meet the WHO (2014) criteria (sensitivity of ≥80% and specificity of ≥98% for EPTB specimens) based on high‐priority target product profile. Although quantitative RT‐I‐PCR employed in this study could monitor the progression of disease, no substantial improvement in sensitivity was observed by RT‐I‐PCR when compared with I‐PCR.…”

“…2018; Yu et al . 2020). The present findings and the previous reports recommend that Xpert could be used as an excellent rule‐in test for diagnosing OATB cases but not as a promising rule‐out test.…”

“…The GeneXpert system, capable of rapidly identifying MTB with rifampin (RIF) resistance within 2 h, has been widely investigated using a wide variety of samples, including paraffin-embedded tissues ( 11 ), blood samples ( 19 ), sterile body fluids ( 20 ), bones ( 21 ), and extrapulmonary samples ( 13 , 22 ), thus proving that GeneXpert is a valuable tuberculosis diagnostic tool. In comparison, the culture of MTB requires a longer cycle (usually 14 to 28 days).…”

“…A high index of clinical suspicion is the mainstay to the diagnosis of spinal TB. On biopsy, in addition to culture, molecular testing can be performed with Xpert/RIF sensitivity of 82-95.6% and specificity of 96.2-100% to detect spinal TB [45,46]. Material for detection of M. tuberculosis can be acquired via bone biopsy, with limited availability in resource-limited settings.…”
