Perforated Mitral Valve Aneurysm in the Posterior Leaflet without Infective Endocarditis

Abstract: Aneurysm of the mitral valve, although uncommon, occurs most commonly in association with infective endocarditis of the aortic valve and true mitral valve aneurysm is a rare cause of mitral regurgitation. We report a case with perforated mitral valve aneurysm in the posterior leaflet without concurrent infective endocarditis initially mistaken diagnosis of cystic mass, which was confirmed at operation with successful mitral valve annuloplasty.

“…Previous studies have reported a conservative nonsurgical management. However, it does appear surgical intervention is warranted in cases of ruptured aneurysms with severe valvular regurgitation or heart failure . Based on our histologic findings, mitral valve aneurysms appear to be a result of an intravalvular abscess which has expanded.…”

“…Direct infection processes have been described, as occurred with the third case in our cohort. However, it is worth mentioning that in this type of presentation, the degenerative compromise of the valve may be more frequent, just as some clinical case records have reported 9, 10…”

“…L'anévrysme de la valve mitrale est souvent associé à une endocardite infectieuse. Les rares cas en dehors de ce contexte ont une des pathologies suivantes: connectivites, dégénérescence valvulaire myxœdémateuse, pseudoxanthome élastique ou lors d'une insuffisance aortique sévère [6]. Dans notre cas, le patient a une endocardite infectieuse certaine aortique et mitrale.…”
