Meckel’s diverticulum is the most common gastrointestinal tract anomaly. It arises from the incomplete closure of the omphalomesenteric conduit, which is a true diverticulum at the antimesenteric border of the ileum. Although the majority of patients are asymptomatic, they can present with inflammation, hemorrhage, intussusception, intestinal obstruction, and perforation, among others; this constitutes an important differential diagnosis for acute abdomen. A 19-year-old female sought medical attention because of intermittent diffuse abdominal pain for two months, nausea, and diarrhea. In the requested imaging tests, tomography, and enterotomography, a diagnosis of Meckel’s diverticulum with some degree of intussusception was suggested. The patient underwent elective surgical treatment without complications and was discharged on the second postoperative day with clinical improvement. In this section, we review publications on similar cases published in the last five years.

ANTECEDENTES: El divertículo de Meckel es la anomalía congénita más común del aparato gastrointestinal, se encuentra en alrededor del 2 al 3% de la población. Se estima que, incluso, el 50% de los divertículos contienen tejido ectópico, de los que un 60 a 85% es tejido gástrico y del 5 al 16% corresponden a tejido pancreático. Gran parte de los divertículos son asintomáticos (cerca del 95.2%) y dependen del tipo de complicación del divertículo. CASO CLÍNICO: Recién nacido a las 32 semanas de gestación, con peso al nacimiento de 1050 g y talla de 36 cm, con dificultad respiratoria. A su ingreso a la unidad de cuidados intensivos neonatales, a las 19 horas de vida, fue intubado, sin datos de problema abdominal. En la radiografía de tórax pudo apreciarse la parte superior del abdomen con neumoperitoneo; por eso se decidió la laparotomía exploradora en la que se identificó el divertículo de Meckel perforado, a 40 cm de la válvula íleo-cecal, que se intervino mediante un procedimiento laparoscópico. CONCLUSIÓN: El divertículo de Meckel sintomático y la manifestación clínica de su perforación es rara en el periodo neonatal; por esto difícilmente se tiene sospecha diagnóstica de forma prequirúrgica. El proceso diagnóstico y terapéutico más adecuado es similar al de otras causas de perforación intestinal: la laparoscopia.

Perforated Meckel’s diverticulum in neonates: a report of six cases and systematic review of the literature

Background Perforation of Meckel Diverticulum (MD) is a rare cause of pneumoperitoneum in neonates. We hereby report six cases of perforation of MD in neonates, with addition of 53 cases from systematic review of the literature. A systematic review was performed using Mesh terms “Neonate, Meckel Diverticulum, Perforation, Pneumoperitoneum.” All reports of perforated MD in the English literature were identified. Details of our 6 cases were analyzed in similar fashion. Results A total of 3027 manuscripts were screened and 59 cases including 6 of our own were identified. The vast majority (78%) were female. Fifty patients (84.7%) presented in the newborn period. Half of the cases (52.5%) had associated anomalies and 13 neonates (22%) required oxygen supplementation including CPAP or ventilatory support before surgery. In 73% of the cases, a resection of gut was undertaken. Histopathological assessment in 44 cases (74.6%) revealed no ectopic gastric mucosa. Three cases demised prior to treatment. The outcome in the vast majority was excellent with 84.7% surviving and discharged well. Conclusion Perforated MD is an unusual cause of a pneumoperitoneum in the newborns. Diagnosis is established at laparotomy and it rare to find ectopic mucosa histopathologically. The overall outcome is excellent.
