Percutaneous vertebroplasty in vertebral osteonecrosis (Kümmell's spondylitis)

Abstract: The authors report the clinical symptoms and response to therapy of a series of patients who presented with subacute or chronic back pain due to vertebral osteonecrosis (Kümmell's spondylitis) and who underwent percutaneous vertebroplasty. The authors performed a retrospective chart review of a series of 95 patients in whom 149 painful, nonneoplastic compression fractures were demonstrated and who were treated with percutaneous transpediculate polymethylmethacrylate (PMMA) vertebroplas… Show more

“…For acute vertebral collapse with axial back pain, minimally invasive procedures, such as vertebroplasty, are attractive and effective options for pain relief and enable patients to mobilize [10,11]. Vertebroplasty for vertebral fracture with or without intravertebral clefts have yielded similar results in terms of pain relief [12,13].…”

“…The intravertebral cleft was a confluent reservoir and specifically targeted for polymethylmethacrylate (PMMA) (Osteobond; Zimmer, Warsaw, IN) injection. Before PMMA injection, cavitygram of the IVC was performed using nonionic contrast agent (Omnipaque 300; Nycomed, Princeton, NJ) to exclude needle placement within the basivertebral venous complex [6], and measured the capacity of the IVC. Persistent opacification could obscure visualization of the PMMA.…”

“…Osteonecrosis is also described as vascular necrosis, pseudarthrosis, and Kummel's spondylitis, 6,7,[11][12][13][14][15][16][17] which are all different names for the same disease. On radiography or computed tomography, it typically manifests as an intravertebral cleft or 'gas sign' with fluid and gas in the intravertebral body (Fig.…”

“…When osteonecrosis and pseudarthrosis were evident with vertebra plana, 4,5 vertebroplasty was considered more suitable. 6,7 We retrospectively reviewed and compared the efficacy of balloon kyphoplasty for vertebra plana with or without osteonecrosis.…”
