IntroduçãoNos procedimentos endovasculares, sejam diagnósticos ou intervencionistas, o sítio arterial de acesso mais comum é a artéria femoral 1 . Na atualidade, a frequência destes procedimentos vem aumentando, porém, sem resultar num aumento da incidência de complicações pós-procedimento 1 . Dentre as principais complicações podemos citar o pseudoaneurisma arterial, hematoma, infecção e fistula arteriovenosa 1 , elevando consideravelmente a morbidade destes pacientes. Os tratamentos podem ser conservador ou procedimentos invasivos, sejam cirúrgicos, sejam percutâneos, tanto para os pseudoaneurismas, quanto para as fístulas arteriovenosas. A ocorrência concomitante de PSA e FAV é condição extremamente rara 2 , tendo sido descrita poucas vezes na literatura. O objetivo deste estudo é relatar um caso de pseudoaneurisma femoral com fístula arteriovenosa associada após procedimento endovascular e sua abordagem terapêutica. Relato do CasoTrata-se de paciente de 68 anos, masculino, portador de hipertensão arterial sistêmica e diabetes mellitus tipo 2, submetido a ablação devido à fibrilação atrial, sendo realizado punção da artéria femoral direita para realização do procedimento. Evoluiu uma semana após punção com hematoma e desconforto em região femoral direita. Foi realizado ultrassom com Doppler colorido (UDC) que evidenciou um pseudoaneurima (PSA) de grande dimensão em artéria femoral superficial com colo de aproximadamente 0,5cm (Figura 1) associado à fístula arteriovenosa (FAV) entre artéria e veia femorais superficiais, com dois orifícios fistulosos estando o mais proximal a aproximadamente 3cm da bifurcação femoral (Figuras 2 e 3 uso de anticoagulante. Assim, como o PSA era mais profundo, de grande dimensão e colo amplo foi optado pelo tratamento com injeção de trombina guiada por UDC para fechamento do PSA. Ao término do procedimento, foi evidenciado material misto (hipo e hiperecogênico) em seu interior e ausência de fluxo ao UDC (Figura 4). Realizado curativo compressivo e encaminhado o paciente para a enfermaria. Foi realizado novo estudo ecográfico e comprovado o sucesso do procedimento. Recebeu alta em uso de anticoagulante.Para a abordagem da FAV foi optado pelo acompanhamento clínico na expectativa de que houvesse fechamento espontâneo. Após um período de acompanhamento de três meses, não houve regressão da FAV, sendo optado pelo tratamento percutâneo para correção. Através do acesso femoral esquerdo retrógrado (via contra-lateral), realizouse o crossover da aorta e feito uma angiografia do referido sítio. Arteriografia corroborou aos achados do UDC: dois orifícios fistulosos em artéria femoral superficial. Realizado a ultrapassagem dos orifícios com cateter MP e fio guia 0,035" e realizado o implante do stent em femoral superficial, ocluindo os orifícios fistulosos (Figuras 5 e 6). A 24-year-old woman presented with neck bruit and thrill. Neurological examination revealed no focal abnormality. Angiography and magnetic resonance imaging showed an arteriovenous fistula (AVF) at the C4-5 levels of the right vertebral artery (VA). The fistula had a single orifice that drained into the epidural veins via the C4-5 intervertebral foramen. A stent-graft was made of a 3.9 cm balloon-expandable stent and an expandable polytetrafluoroethylene graft sutured outside the stent. The stent-graft was advanced through the femoral artery up to the VA but did not pass far enough distally to seal the entire fistula because of the high friction between the bulky device and the small vessel. Another 2 cm stentgraft was delivered and successfully positioned across the fistula. The AVF completely disappeared and the right VA was well preserved. Follow-up angiography at 3 weeks and 5 months after the treatment showed no fistula and no stenosis inside the stent-grafts. Stent-grafts are useful to treat AVF with a large orifice. The stent-graft is a promising technology to treat neurovascular diseases, although improvement is required for use in smaller vessels.
