Percutaneous transhepatic biliary drainage catheter tract recurrence in cholangiocarcinoma

Abstract: PTBD catheter tract recurrence is not unusual. The prognosis for these patients is generally poor, even after resection. To prevent this troublesome complication, endoscopic biliary drainage is first recommended when drainage is indicated.

“…Meta-analyses comparing EBD with PTBD in patients with malignant biliary obstruction have reported that PTBD has a higher success rate, with no significant difference in complications as compared with EBD [6]. However, because PTBD carries the risk of tumor cell dissemination [7], it is unsuitable for surgical patients and is difficult to perform in patients with ascites or Chilaiditi's syndrome. On the other hand, although EBD has been reported to have a low success rate, bile flow is physiological if EBD is successful, thereby contributing to maintaining a better quality of life than an external fistula.…”

“…However, it should be noted that catheter tract recurrence occurred in 5-6% of cases within patients who received PTBD [66,67]. PTBD only for cytological or histological analysis is not recommended.…”

“…39,40 Currently, there are no significant differences in sensitivity and complications between cytology/biopsy with ERCP or PTC, but more attention should be paid to the fact that catheter tract implantation metastasis is not a rare complication following PTBD in ECC. 41,42 The study of Kim et al 43 indicated that PTC in combination with IDUS was highly accurate for assessing Bismuth type in patients with hilar CCA, which may help in the identification of an optimal surgical plan for the treatment of hilar CCA, especially in Bismuth type III and IV. IDUS images also have important clinical significance in the differentiation of malignant and benign lesions.…”
