ÔÃÁÓ Ôåäåðàëüíûé íàó÷íî-êëèíè÷åñêèé öåíòð ñïåöèàëèçèðîâàííûõ âèäîâ ìåäèöèíñêîé ïîìîùè è ìåäèöèíñêèõ òåõíîëîãèé ÔÌÁÀ Ðîññèè, ÌîñêâàСтатья посвящена актуальной проблеме диагностики и лечения больных с механической желтухой различного генеза. Основное внимание авторы уделяют использованию малоинвазивных методов декомпрессии билиарной системы: эндоскопических и чрескожных. Подробно описаны эффективность, показания и противопоказания для каждого малоинвазивного метода. Авторы предлагают свой алгоритм использования малоинвазивных методик в диагностике и лечении больных с механической желтухой.Ключевые слова: малоинвазивные эндоскопические и чрескожные вмешательства, механическая желтуха, билиарная система. METHODS OF DIAGNOSTICS AND TREATMENT OF PATIENTS WITH MECHANICAL JAUNDICEThe article is dedicated to actual problem of diagnostic and treatment of patients with mechan ical jaundice of different genesis. The authors consider that using of low invasive methods of biliar decompression -endoscopic and transcutaneous -is of great importance now. It is described effi ciency, indications and contraindications for each low invasive method in details. The authors offer their own algorithm of diagnostic and treatment in patients with mechanical jaundice.

Механическая желтуха сопровождается нарушением внешнесекреторной функции печени, значительным увеличением в сыворотке крови концентрации биологически активных компонентов желчи -общих желчных кислот и, как следствие, развитием эндотоксикоза.Цель исследования -дать оценку ультраструктурным изменениям, развивающимся в эпителиоцитах проксимальных канальцев нефронов почек крыс в динамике экспериментального подпеченочного обтурационного холестаза.Материал и методы. Экспериментальный подпеченочный обтурационный холестаз (продолжительностью 1. 3, 10, 30 и 90 суток) у крыс моделировали путем перевязки общего желчного протока в области ворот печени. Контролем служили ложнооперированные животные. В сыворотке крови опытных и контрольных крыс энзимо-колориметрическим методом определяли концентрацию общих желчных кислот. Для электронно-микроскопических исследований почек крыс использовали электронный микроскоп JEM-100 CX II фирмы "JROL" (Япония).Результаты. У холемичных животных на протяжении всего периода наблюдений в эпителиоцитах проксимальных канальцев корковых и юкстамедуллярных нефронов развиваются ультраструктурные изменения, сопровождающиеся дезорганизацией микроворсинок щеточной каемки, очаговым разрушением апикальной поверхности эпителиоцитов, нарушением структуры митохондрий, увеличением содержания лизосом, развитием гидропической дистрофии.Заключение. Наблюдаемые изменения являются прямым доказательством тяжелых морфологических изменений в канальцевом аппарате нефронов, степень выраженности которых зависит от продолжительности холестаза и, соответственно, эндогенной интоксикации, обусловленной холатемией.Ключевые слова: холестаз, эндогенная интоксикация, эпителиоциты, проксимальные канальцы, нефроны. Treatment of patients with mechanical jaundice (MJ) has been one of the topical problems of current medicine. The fraction of patients having MJ of oncologic character that can undergo radical surgery does not exceed 25-30%. A way out of such situation is to use minimally invasive endobiliary interventions, the main type of which is endoprosthesis replacement of bile duct lumens. The aim of the work was to study the biological properties of the experimental models of polymer stents made of PHA for endobiliary prosthetics and to investigate the biological properties of PHA suture material for forming of biliary-enteric anastomoses (cholecystoduodenostomy). Experiments were performed on 20 adult mongrel dogs, weighing 10-12 kg. The animals were divided to three groups: the negative control group (intact animals); the positive control group (the animals with implanted endobiliary silicon stents); and the experimental group (the animals with the PHA stents). The animals were monitored for 100 days. The clinical blood analysis was made before the operation, on day 7, 30, 60 and 100 days. During autopsy the presence of exudate, commissural process in the free abdominal cavity and subhepatic spatium, the appearance of choledoch where the prosthesis was located, as well as the appearance of cholecystoenterostomy, liver and duodenum. We did not found any signs of inflammation, cicatrical changes were estimated in the free abdominal cavity and subhepatic spatium. All implanted PHA stents were at their initial places of implantation. After the end of the experiment inflammatory reaction and anastomositis were absent. Macroscopic changes of liver and duodenum were also not detected. Liver function did not have any pathological deviations. These positive results give grounds to conclude that application of PHA as endobiliary stents in reconstructive surgery of bile passages and as suture material is a promising technology.
