“…The techniques presented in the present study were modifications of the standard posterolateral approaches to the L5-S1 lumbar spine under fluoroscopic guidance. Percutaneous nucleotomy 6,14 was applied as a main procedure to remove extruded material, and automated percutaneous lumbar discectomy 1,8,9,11 as a supplement to further clear any residual material.…”

“…Compared with the posterolateral L5-S1 discectomy and automated percutaneous lumbar discectomy, 1,6,8,9,14 the advantages of the anterior approach include the following: 1) avoidance of paraspinal muscle trauma and partial denervation; 2) removal of more of the offending extruded disc fragments, which in turn minimizes the chance of recurrent disc herniations at the affected level; 3) less postoperative pain; and 4) earlier hospital discharge. 3,4,12 In addition, the TPLD procedure is not so technically demanding; an access operator experienced with lumbosacral anatomy could successfully perform the procedure and minimize potential complications.…”

“…The principle of mechanical nucleotomy was subsequently pursued by other investigators. [41,49] In the early 1980s, [21,26] in an attempt to reach the herniated fragments that were dislodged under the posterior longitudinal ligamentum, the Craig instruments were modified and with the diameter of the cannula enlarged (5 mm inside diameter [ID], 6.9 mm outside diameter [OD]). This allowed the passage of upbiting and deflecting instruments.…”
