This systematic review aims to illustrate the published results of "minimally invasive" procedures for correction of hallux valgus. Based on former systematic reviews on that topic, the literature search was organised by two independent investigators. MEDLINE was systematically searched for available studies. The keywords used were "hallux valgus", "bunion", "percutaneous surgery", "minimally invasive surgery", "arthroscopy", "Bosch" and "SERI". Studies were assessed using the level of evidence rating. A total of 21 papers were included in this review. These studies described a total of 1,750 patients with 2,195 instances of percutaneous, minimally invasive or arthroscopic hallux valgus surgery. Clinical reports of results after minimally invasive hallux valgus surgery at meetings are common. Published results in peer-reviewed journals are less common and the majority of papers are level IV studies according to the level of evidence ratings. We found one level II and three level III studies. Reported complications seem to be less than one may see in one's own clinical practice. This possible bias may be related to the fact that most studies are published by centres performing primarily minimally invasive hallux valgus surgery.

Background Ideal surgical treatment for hallux valgus is still controversial. A traditional distal metatarsal osteotomy with rigid fixation (Scarf procedure) and a more minimally invasive approach to a distal metatarsal osteotomy, termed SERI (Simple, Effective, Rapid, Inexpensive), have proven successful with short-term followup. However, no data are available directly comparing the two procedures. Questions/Purposes We performed a prospective randomized trial to determine which technique (SERI or Scarf) was associated with (1) better functional outcomes, (2) better radiographic correction, and (3) fewer complications at 2 and 7 years followup. Methods Twenty patients, 53 ± 11 years of age, with bilateral hallux valgus, clinically and radiographically similar, underwent bilateral surgery with Scarf on one side and SERI on the other, at random. Clinical (AOFAS score) and radiographic assessments were considered before surgery, and at 7 years followup. Results SERI and Scarf techniques provided correction of the hallux valgus angle, intermetatarsal angle, and distal metatarsal angle in the range of normal. Both led to similar clinically important improvements in the AOFAS.No differences were observed between the groups. All osteotomies healed, and two patients who underwent the Scarf procedure required hardware removal. Reduction of ROM with respect to preoperative was observed in three patients for SERI and three patients for Scarf procedures. Conclusions Scarf and SERI techniques resulted in effective correction of hallux valgus with similar outcomes, however the SERI technique required a shorter skin incision, less surgical time, less expensive fixation device, and was without residual pain attributable to hardware.

Objective: This study presents clinical and radiological outcome of a percutaneous technique for the correction of hallux valgus and lesser toe deformities. Methods:We present a 36 months follow-up series of 32 patients who have been treated with the ReverdinIsham osteotomy for the correction of hallux valgus, and a percutaneous technique for the correction of lesser toes deformities and metatarsalgia. Clinical outcome data were recorded with the AOFAS score. Radiologic evaluation consisted of weight bearing (AP, lateral and Walter-Muller) views pre and postoperatively at 6 weeks, 3, 6, 12 and 36 months after surgery.Results: At three year follow-up, the mean difference of the HVA was 9.2 (p<0.0001), of the IMA was 0.4 (p<6719), and the mean difference of the PASA was 15.9 (p<0.001). The AOFAS rose from 48.4 to 87.6. Most encountered complication was oedema that lasted for 6 months, especially in the patients who underwent the Weil osteotomy of II, III and IV metatarsal bone head for the treatment of metatarsalgia. Conclusion:Many minimal invasive techniques are becoming more and more recognized, with some indisputable advantages but also not free of objective difficulties. We believe that percutaneous distal metatarsal bone osteotomy represents a good option for the treatment of mild-to moderate hallux valgus, lesser toes deformities and metatarsalgia.
