Percutaneous ethanol injection for the treatment of small hepatocellular carcinoma. Study of 95 patients

Abstract: Percutaneous ethanol injection (PEI) was applied to 120 lesions in 95 patients with hepatocellular carcinomas (HCC) smaller than 3 cm in the past 6 years. All main target tumours, in 67 patients who had been followed by sonography for more than 6 months after PEI, decreased in size; 28 tumours (41.8%) became undetectable and have remained so until now. The 1-, 2-, 3-, 4- and 5-year survival rates calculated by the Kaplan-Meier method were 93%, 81%, 65%, 52% and 28% respectively. These survival rates were bette… Show more

“…87 Percutaneous alcohol injection is effective in tumors less than 3 cm in size but is far less successful for larger tumors. [88][89][90] Other treatment modalities, such as chemotherapy and tumor embolization, may be useful in individual patients but have not been shown to improve survival in patients with HCC. 90,91 The early results of liver transplantation for nonresectable HCC were quite discouraging.…”

“…However, many patients have poor hepatic reserve owing to underlying chronic liver disease; therefore, only 5% to 40% of the patients with HCCs are candidates for hepatectomy. 4 Various local ablation therapies such as percutaneous ethanol injection 5,6 or percutaneous radiofrequency ablation (RFA) have been proposed as nonsurgical treatment options for small-sized HCCs, and excellent survival rates have been reported with these treatments. 7,8 Complete necrosis was achieved in 76% to 100% of the treated small-sized HCCs.…”

“…Percutaneous ethanol injection (PEI) is now widely carried out for small HCC. [15][16][17][18][19][20][21] The effect of PEI is almost comparable with that of surgical resection. 5,6,[16][17][18][19] However, a large number of treatment sessions are required [15][16][17][18][19][20] and recurrence after treatment is very common.…”
