ÖZETAmaç: Transkateter patent duktus arteriyozus (PDA) tedavisi sonrası sol akciğer perfüzyonun da bozulma daha önce bildirilmiştir. Bu çalışmada PDA tedavisinde sıkça kullanılan kontrollü salınımlı Cook koil ve kontrolsüz salınımlı Gianturco koil lerin sol akciğer perfüzyona etkisinin radyonüklit yöntemle değerlendirilmesi amaçlanmıştır. Yöntemler: Bu prospektif çalışmada 49 olguda Cook koil ve 40 olguda Gianturco koil kullanılmak üzere toplam 89 olgunun transkateter yöntemiyle PDA'sı kapatıldı. Olgular PDA tedavisinden medyan 18 ay sonra akciğer perfüzyon sintigrafisi yapıldı. Azalmış sol akciğer perfüzyonunu öngörebile-cek parametreler incelendi. Akciğer sintigrafisi kantitatif değerleri ile hasta parametrelerinin istatistiksel ilişkisi Pearson korelasyon katsayısı ile hesaplandı, düşük akciğer perfüzyonunu predikte eden değişkenlerden optimal kestirim değeri bulabilmek için 'Receiver Operating Characteristic' (ROC) eğrisi analizi kullanıldı. Bulgular: Sol akciğer perfüzyonunda bozulma 10'u Cook koil kullanımı sonrası, 3 olgu Gianturco koil kullanımı sonrası olmak üzere toplam 13 olguda saptandı (p=0.077). Tüm olgular ele alındığında pulmoner arter tarafında bırakılan sarmal sayısı, koil/duktal çap oranı ve birden fazla koil kullanımı ile ilişkilidir (sırasıyla, p=0.002, p=0.006 ve p=0.009). Pulmoner arter tarafındaki sarmal sayısı 1.4 üzerindeki olgularda %77 özgüllük ve %85 duyarlılık ile ilk koil çapı/duktus en dar çapı oranı 1.85 üzerindeki olgular %70 özgüllük ve %77 duyarlılık ile akciğer perfüzyon sintigrafisinde bozulma öngörü-lebilmektedir (ROC eğrisi altındaki alanlar sırasıyla, 0.804, %95GA 0.661-0.947, p<0.001 ve 0.747, %95GA 0.423-0.790, p<0.005). Sonuç: Transkateter PDA tedavisi sonrası sol akciğer perfüzyon bozukluğu Cook koil ya da Gianturco koil kullanımı sonrası gelişebilir. Bu durum pulmoner arter tarafında bırakılan sarmal sayısı, duktus en dar çapına göre koil'in daha büyük seçilmesi ve birden fazla koil kullanımı ile ilişkilidir. ABSTRACTObjective: Impaired left lung perfusion (LLP) has been described after transcatheter closure of the patent ductus arteriosus (PDA). In this study, we aimed to evaluate lung perfusion scans (LPSs) following occlusion of PDA with two frequently used coils: Cook detachable coil and Gianturco coil. Methods: A prospective study of 89 patients who underwent PDA occlusion using the Cook coils in 49 and the Gianturco coils in 40, was conducted. LPSs were performed after a median period of 18 months. The relationships between the LPSs and continuous variables were assessed by Pearson correlation analysis and the cut-off value of the best correlated parameters to predict abnormal LPSs obtained by the Receiver Operating Characteristic (ROC) analysis. Results: Decreased left lung perfusion (LLP) was found in 13 patients including 10 after using Cook coils and 3 after using Gianturco coils (p=0.077). We describe a novel method that allows bioptome-assisted delivery of multiple Gianturco coils simultaneously for occlusion of the large patent ductus arteriosus (PDA). Two or more coils were intertwined at one end and held by a bioptome (5.2 Fr) and pulled into a short introducer. The coils were then deployed in the PDA via a long sheath (7-11 Fr) previously placed across the duct via the femoral vein. Twelve patients (6 months to 64 years; median, 10.5 years) with large PDAs (4.7 +/- 1.3 mm; range, 3.1-8.4 mm; PA mean pressure, 40 +/- 17 mm Hg; pulse pressure 63 +/- 18 mm Hg) underwent bioptome-assisted occlusion with multiple coils at our institutions. The procedure was uneventful in nine patients (fluoroscopy time, 6-23 min) and prolonged in three patients (fluoroscopy time, 26, 72, and 120 min) because of dislodgment of the coil mass and embolization of an additional coil. Successful coil deployment was feasible in all patients. Three patients required repeat coil deployment for flow elimination (hemolysis occurred in two). Flow elimination was demonstrated on the last follow-up evaluation in all except two patients. One infant has developed significant left pulmonary artery stenosis. Bioptome-assisted PDA occlusion using multiple coils delivered simultaneously may be a promising alternative to devices for transcatheter closure of large PDAs. SUMMARYTo assess the influence of ductal size on the results of transcatheter closure of patent ductus arteriosus (PDA) with coils, 154 consecutive patients were studied prospectively. Ductal size was defined as the narrowest diameter of ductus measured on aortography. All patients were divided into 5 groups according to ductal size: < 1 mm, 1-1.9 mm, 2-2.9 mm, 3-3.9 mm, and ≥ 4 mm. The occlusion of PDA with coils was performed through a transarterial approach. The results were evaluated by angiography at 10 minutes and by color Doppler echocardiography at 1 day, 2 days, 1 week, 1 month, 3 months, 6 months, and 12 months after the procedure. The immediate occlusion rates for ductal sizes < 1 mm, 1-1.9 mm, 2-2.9 mm, 3-3.9 mm, and ≥ 4 mm were 89.7%, 75.4%, 51.4%, 30.8%, and 40%, respectively; whereas the occlusion rates at 12-months follow-up were 100%, 98.5%, 97.3%, 69.2%, and 80%, respectively. There were no significant differences in occlusion rate at 12-months follow-up among the groups with ductal sizes < 3 mm or among the groups with ductal sizes ≥3 mm. The occlusion rate for ductal size < 3 mm at each follow-up time was significantly higher than that for ductal size ≥ 3 mm (10 minutes: 71.8% vs 34.8%, P = 0.001; 12-months: 98.5% vs 73.9%, P < 0.001). The occlusion rate of residual shuntings at 12-months follow-up for ductal size < 3 mm was also significantly higher than that for ductal size ≥ 3 mm (94.6% vs 60%, P = 0.007). The results of the present study demonstrate that ductal size < 3 mm had a higher occlusion rate than that for a size ≥ 3 mm. PDA with a size ≥ 3 mm may need other treatment strategies or other devices to achieve better results. (Jpn Heart J 2003; 44: 395-401)
