Akut aşil tendon rüptürü (ATR) tedavisi halen tartışmalı bir konudur. Geleneksel açık cerrahi tedaviye göre daha düşük re-rüptür oranları ve daha az yara yeri sorunları gözlenmesi nedeniyle akut ATR'nin mini-açık ve perkütan olarak uygulanan minimal invaziv tekniklerle tedavisi daha iyi bir seçenektir. Bu teknikler ile başarılı sonuçlar alınmasına rağmen, sural sinir yaralanması bu tekniklerin en önemli komplikasyonudur. Bu çalışmada, akut ATR tedavisinde kullanılan minimal invaziv tekniklerin (mini-açık ve perkütan tamir) klinik sonuçlarının ve komplikasyon oranlarının karşılaştırılması amaçlanmıştır. Gereç ve Yöntem016-2019 yılları arasında akut ATR'si minimal invaziv yöntemler ile tamir edilen toplam 42 hasta çalışmaya alınmıştır. Hastalar, cerrahi kesiden tendon uçlarının gözlenebildiği mini-açık tamir grubu (Grup 1, n=22) ve gözlenemediği perkütan tamir grubu (Grup 2, n=20) olmak üzere 2 gruba ayrıldı. Klinik değerlendirme için AOFAS skoru, ayak bileği plantar fleksiyon ve dorsifleksiyon açıları, işe ve spora geri dönüş süreleri kullanıldı. Cerrahi sonrası komplikasyonlar ise minör (tekrar cerrahi gerektirmeyen) ve majör (tekrar cerrahi gerektiren) komplikasyonlar olmak üzere iki kategoriye ayrıldı. Gruplar fonksiyonel sonuçlar ve komplikasyonlar açısından karşılaştırıldı. BulgularHastaların ortalama yaşı 40,30±5,56 (23-50) olarak bulundu. Gruplar arasında yaş, cinsiyet, ameliyata kadar geçen süre, ameliyat süresi, takip süresi açısından istatistiksel olarak anlamlı bir fark bulunamadı (hepsi için p>0.05). Hastaların ameliyat sonrası ikinci yıl fonksiyonel sonuçları ile işe ve spora dönüş süreleri açısından gruplar arasında istatistiksel olarak anlamlı bir fark bulunamadı (hepsi için p>0.05). Mini-açık grupta 2 hastada ve perkütan tamir grubunda 3 hastada minör komplikasyon gözlendi. Sural nöropati perkütan tamir sonrası 3 (%15) hastada görülürken, mini-açık tamir sonrası hiçbir hastada görülmedi. Her iki grubun birer hastasında tekrar ameliyat gerektiren major komplikasyon görüldü. Gruplar arasında hem minör hem majör komplikasyonlar açısından istatistiksel anlamlı fark bulunamadı (sırasıyla p=0,566 ve p=0,947). SonuçAkut ATR'nin mini-açık veya perkütan tamirle tedavisinin fonksiyonel sonuçları ve komplikasyonları benzerdir. Daha düşük sural sinir yaralanma riski olması nedeniyle mini-açık tamir yöntemi daha avantajlı olabilmektedir.

Introducción: Las técnicas quirúrgicas modernas buscan rehabilitar en forma temprana para una recuperación en menor tiempo. Las técnicas mínimamente invasivas son una opción para lograrlo. Objetivo: Evaluar la capacidad de comenzar una rehabilitación y la carga de peso en el posoperatorio inmediato utilizando una técnica mínimamente invasiva sin nudos y la evolución a los 6 meses de la cirugía. Materiales y Métodos: Se realizaron 10 reparaciones quirúrgicas de lesiones agudas del tendón de Aquiles con una técnica mínimamente invasiva sin nudo. Se utilizó el sistema combinado (PARS – SpeedBridgeTM original. A las 48 h se permitió la carga completa de forma progresiva, según tolerancia. Se evaluó el resultado final a los 6 meses utilizando el puntaje ATRS. Resultados: En promedio los pacientes tardaron 14 días en deambular sin muletas. Todos comenzaron la rehabilitación con movilidad activa y pasiva del tobillo durante la primera semana posoperatoria. El puntaje ATRS promedio fue de 79 (rango 60-90). Conclusión: Esta técnica ha demostrado ser una excelente opción quirúrgica en las lesiones agudas del tendón de Aquiles de media sustancia, pero no fue posible comenzar con la carga de peso inmediata debido al dolor o temor del paciente, pero sí con la rehabilitación dentro de la primera semana posquirúrgica.

Objective Percutaneous suture is a classic technique used in Achilles tendon repair. However, the complication rates surrounding the sural nerve remain relatively high. Modified percutaneous repair technology can effectively avoid these complications; however, the surgical procedure is complicated. Hence, the present study was conducted to describe a redesigned repair technique for the Achilles tendon able to avoid sural nerve injury and reduce the complexity of the procedure. Methods Data of patients with acute primary Achilles tendon rupture at our hospital from January 2019 to May 2020 were included. Subjects with expectations for surgical scarring underwent a minimally invasive‐combined percutaneous puncture technique. The surgical time, requirement for conversion to other technologies, and length of postoperative hospitalization were investigated to assess efficacy. The American Orthopedic Foot & Ankle Society (AOFAS) score and the Arner–Lindholm scale (A‐L scale) were used to assess postoperative clinical outcomes (> 24 months). During the 2‐year follow‐up, MRI was performed to observe the healing of the Achilles tendon. In addition, subjective satisfaction with surgical scar healing was recorded. Results Twenty consecutive subjects with an average follow‐up of 28.3 ± 4.5 months (range, 24–41) met the inclusion criteria. None of the 20 enrolled patients required a converted surgical approach. The mean surgical time was 26.9 ± 6.47 min (range, 20–44). None of the patients experienced dysesthesia or anesthesia around the sural nerve. No signs of postoperative infections were observed. MRI data showed that the wounds of the Achilles tendon healed completely in all the subjects. The AOFAS score increased from 55.6 ± 11.07 (range, 28–71) preoperatively to 97.8 ± 3.34 (range, 87–100) at the last follow‐up. The A‐L scale showed that 90% of the subjects (n = 18) presented as excellent and 10% of the subjects (n = 2) presented as good, with an excellent/good rate of 100%. Moreover, subjects' satisfaction for surgical scars was 9.1 ± 0.78 (upper limit, 10). Conclusions The results indicate that this technique can achieve good postoperative function, a small surgical incision, and high scar satisfaction. In addition, this technique should be widely used in suturing Achilles tendon ruptures.
