PERCIST in Perspective

Abstract: Positron Emission tomography Response Criteria In Solid Tumors (PERCIST) version 1.0 was introduced in 2009 for objective assessment of tumor metabolic response using 18 F-FDG PET/CT. Practical PERCIST: A Simplified Guide to PET Response Criteria in Solid Tumors 1.0 was published in 2016 to review and clarify some of the issues with the PERCIST. In this article, we reflect on the benefits and challenges of implementing PERCIST, and speculate on topics that could be discussed in PERCIST 1.1 in the future.

“…It is simple and quick to measure but is subject to noise because its value is taken from a single voxel. Alternatively, SUV mean, median or standard deviation within a VOI bounded by a threshold SUV may be more robust [17]. SUV thresholds for bounding VOI may be absolute (e.g.…”

“…Volumetric PET parameters include the MTV, a measure of metabolic tumour volume above a threshold SUV, and total lesion glycolysis (TLG), which is the product of the mean SUV and MTV. Many feel that MTV and TLG better reflect metabolic tumour burden [17] and studies that have correlated MTV and TLG with prognosis [1820] have included both cervical and head and neck cancer treated with CRT [21, 22].…”

“…For example, a recent study suggested that FDG-PET/CT performed 14 days after the start of neo-adjuvant chemotherapy in patients with locally advanced disease was able to identify patients with poor outcomes, based on an increase in regional lymph node maximum SUV and insufficient decrease in primary tumor uptake after 2 weeks of treatment (47). Currently, the PERCIST thresholds (decrease ≥30%) do not vary according to the number of treatment cycles received (i.e., mid-treatment or end-oftreatment), which may change in subsequent versions (48). Specifically for head-and-neck cancer response assessment, Marcus et al proposed the Hopkins 5-point interpretation criteria to assess locoregional response after chemoradiotherapy.…”

“…A further issue is the standardization of acquisition and quantification procedures. This has been addressed by several authors in papers about PERCIST [5,6,29,30]. Scanners and reconstructions methods are under continuous technical improvement [29] to improve not only the quality of imaging but also the quantitative measures.…”
