Penile Reconstruction with Bilateral Superficial Circumflex Iliac Artery Perforator (SCIP) Flaps

Abstract: The free radial forearm flap is a very common material for penile reconstruction. Its major problems are donor-site morbidity with large depressive scar after skin grafting, urethral fistula due to insufficiency of suture line for the urethra, and need for microvascular anastomosis. A new method using combined bilateral island SCIP flaps for the urethra and penis is developed for gender identity disorder (GID) patients. The advantages of this method are minimal donor-site morbidity with a concealed donor scar,… Show more

“…The SCIP flap is commonly used in the form of a free flap, and there are few reports discussing the application of the SCIP as a propeller type. These reports are especially rare for perineal reconstruction (Boissière et al, ; Han et al, ; Koshima et al, ; Luca‐Pozner et al, ). When the SCIP flap is used as a free flap, it does not need to be buried in a subcutaneous tunnel following rotation.…”

“…Free flap urethral reconstruction has been described predominantly in oncological cases, but also as salvage option after partial necrosis of the neourethral part of a tube‐in‐tube phalloplasty (Camp, Cartwright, & Siddiqi, ; Dabernig, Shelley, Cuccia, & Schaff, ; Lee, Hur, Song, & Tark, 2001; Tchang et al, ). Single‐stage double flap phalloplasty with free or pedicled flap urethral reconstruction has been described only in case reports and small case series (Harashina et al, ; Koshima et al, ; Yoo, Shin, and Lee, ). For urethral reconstruction, use of the RFFF, gracilis flap, ulnar flap and pedicled superficial circumflex iliac artery perforator flap (SCIPF) were described.…”

“…They are used in a variety of reconstructions, including limb, head and neck, external auditory canal, and pudendal reconstruction. 69 Although not as commonly used as anterolateral thigh flaps and deep inferior epigastric artery perforator flaps, its merits include its thinness, short surgical duration, and less invasiveness at the donor site. These characteristics make the SCIP flap especially suitable for dorsalis pedis reconstruction.…”
