Penile Fracture in Kermanshah, Iran: Report of 172 Cases

Abstract: Patient misinformation about penile tissue properties is the main explanation for the high incidence of penile fracture. Cavernosography, and urethrography and intraoperative urethral catheterization are not routinely needed, as diagnosis can be made clinically. Preoperative and postoperative use of antibiotics, and a uniform surgical plan regardless of delay in presentation are recommended.

“…In most cases, injury to the erect penis occurs during intercourse when the erect penis slips out of the vagina and is subsequently thrust against the partner's perineum or pubic bone, commonly while the partner is in the superior position 2 . In the remainder of the cases, injury has been caused by the abdominal bending of the penis during masturbation to achieve detumescence, trying to place the erect penis into pants, rolling over in bed, bumping into furniture in the dark, and a kick from horse 3–7 . Erectile dysfunction (ED) is a major disabling long‐term complication following penile fractures.…”

“…It probably remains under-reported due to the embarrassing nature of the condition. Penile fracture is more prevalent in the Middle East than the Western countries 2 . Only a few cases have been reported in the United States in recent years, and even fewer cases in the United Kingdom.…”
