ÖZETGiriş / Amaç: Travma veya cerrahi prosedürler sonrası yüzeyel epidermal dokunun dermis yada subkutan dokuya implantasyonu sonucu gelişen epidermal kistler, vücudun herhangi bir bölgesinde görülebilen gerçek kistik oluşumlardır. Kistlerin çevresi keratinize skuamöz epitelle çevrili, içi keratin yıkıntısı ile doludur. Bu çalışmada, cerrahi sünnet sonrası gelişen penil epidermal kistli iki olgu sunulmuştur. Olgu 1: 2 yıl önce dış merkezde çocuk cerrahisi uzmanı tarafından yapılan sünnet öyküsü mevcut 4 yaşındaki olguda, bir yıldır fark edilen ve giderek büyüyen penisde kitlesel lezyonu mevcuttu. Penil frenulum bölgesinde sirkümsizyon insizyonunun altında yaklaşık 1cm çapında yuvarlak düzgün konturlu mobil kitlesel lezyon palpe edilmekteydi. Yüzeyel Doku ultrasonografisi, penil frenulum bölgesi cilt altında 10,7x10,4mm solid kitlesel lezyon şeklinde rapor edildi. Doppler ultrason ile kitle çevresinde akım sinyali izlenirken santralinde akım olmadığı tespit edildi. Laboratuvar tetkiklerinde bir özellik saptanmayan olguda operasyonda kitle total olarak rüptüre olmadan çıkarıldı. Kitlenin histopatolojik incelemesinde enfekte epidermal kist saptandı. Olgu 2: Bir yıl önce merkezimizde çocuk cerrahisi uzmanı tarafından yapılan sünnet öyküsü mevcut 2 yaşındaki olguda, yeni farkedilen penil kitlesel lezyonu mevcuttu. Penil frenulumda sünnet insizyonu hizasında 0,6mm çapında yuvarlak şekilli nodüler görüntülü polipoid mobil kitle palpe edilmekteydi. Laboratuvar tetkiklerinde bir özellik saptanmayan olgu opere edildi, kitle total olarak rüptüre olmadan çıkartıldı. Kitlenin histopatolojisi epidermal kist şeklinde raporlandı. Sonuç: Çocukların tekrarlanan psikolojik ve cerrahi travmalara maruz kalmamaları için sünnetin önemsiz ve sıradan bir işlem gibi görülmemesi ve uzman doktor tarafından da özenli bir şekilde yapılması gerekmektedir. Sünnet, sağlam bir vücut parçası üzerine yapılan gerçek bir ameliyattır. Her cerrahi işlem gibi geçerli olan gereken özenin gösterilmesi ile bu ve bunun gibi diğer sünnet komplikasyonlarının önüne geçilebilir. "Ama biz sünneti çocuk cerrahına yaptırmıştık!" şeklinde ifade ettikleri hayal kırıklığını ebeveynlere yaşatmamak temel amaçlarımızdan biri olmalıdır. Systematic review of complications arising from male circumcision

Background: Neonatal male circumcision is the most common procedure performed on paediatric patients (Simpson et al., 2014) and one of the most common surgical procedures in the world (American Academy of Pediatrics, 2012).Methods: A search was conducted for articles about complications arising from male circumcision surgeries by entering the term 'male circumcision' into PubMed on June 16, 2020. Six thousand six hundred forty-one articles published from 1945 to 2020 were found. Seventy-eight articles were ultimately selected for the systematic review.Results: The 78 articles selected from the literature search were entered into one of three tables. The first table includes 15 articles pertaining to chart reviews and cohort studies and report complication rates. The second table reports specific complications from 51 case reports and case series, and the third table is a summary from 12 articles regarding physician questionnaires and society recommendations. Additionally, the 78 articles were used to compile a list of 47 specific complications arising from male circumcision surgeries.Conclusions: Complications from neonatal male circumcisions are common and healthcare providers need to be better informed of the potential complications of the surgery so that they can more effectively counsel their patients about potential risks, likelihood of complications and what can be done to prevent them. While experienced providers who practice in sterile settings have better outcomes with fewer complications, encouraging parents to take into account who is performing their son's circumcision, what was their training, how clean is their practice and how much experience they have and reminding them they have the option to decline the procedure entirely allow the parents to get a more complete picture and play an essential role in the decision-making process.

Steatocystoma simplex in penile foreskin: a case report

BackgroundSteatocystoma simplex is an uncommon skin lesion with a histological pattern that is identical to that of steatocystoma multiplex. We are reporting this case of steatocystoma simplex for its uncommon location in the penile foreskin, and its occurrence in a Wapishana man.Case presentationA 56-year-old man of Wapishana ethnicity presented with complaints of referred penile discomfort and pain during sexual intercourse for 5 years. A physical examination revealed a mobile, compressible subcutaneous non-tender mass of 4 cm diameter located on the left-side of his penile foreskin. There were no signs of inflammation, no grip on the penile shaft, and no urethral discharge or enlargement of lymph nodes. We found no evidence of other cysts on cutaneous examination. We performed classical excision of the lesion under local anesthesia and confirmed the diagnosis of steatocystoma with the pathological report. As there were no complications, we discharged him the same day.ConclusionSteatocystoma can be considered a differential diagnosis for cystic lesions on and around the penis.
