Penile Encrustation Due to a Condom Catheter

Abstract: We report a 68-year-old man with penile encrustation caused by a condom catheter used for urinary incontinence. High-resolution x-ray crystallographic analysis of the encrustation revealed struvite [MgNH4PO4.6H2O] and apatite [Ca5(PO4)3(OH)]. Physicians should be aware that penile encrustation may occur as a complication of condom catheter use. The problem of encrustation is discussed. A brief review of condom catheter complications is presented.

“…The focus of most publications about sheaths has been on the development of clinical problems rather than on their effectiveness. The use of sheaths has been associated with some important complications, such as ulceration [5,6], necrosis [6–8], and severe encrustation [9], particularly in earlier studies when the external fixation strips were in use. UTI has also been found to be associated with sheath use [10,11], but it has been shown that the incidence of UTI amongst catheter users is much higher than that amongst sheath users [12].…”

“…Some SCI patients may develop allergic reactions to the condom. 2 Rare complications of penile sheath drainage are penile encrustation, 3 balanitis, 4 penile urethral diverticulum, 5 intradiverticular lithiasis, 6 bilateral hydronephrosis, 7 and distal penile necrosis. 8 We report a tetraplegic patient in whom a migratory stone from the urinary bladder got impacted in the urethra just proximal to the rim of a penile sheath.…”

“…At the other extreme, your editors continue to marvel at the usefulness of the case report to illustrate geriatric principles. Among our favorites in this regard was the lady who developed dysphoria after cataract removal, 80 the man who suffered penile encrustation due to a condom catheter, 81 and the description of musical hallucinations, “the sounds of silence.” 82 However, the ultimate case report appeared in the Christmas issue of the British Medical Journal 83 . A feline developed self‐neglect after the death of her 89‐year‐old owner.…”
