Dual-energy x-ray absorptiometry (DXA) is used to assess bone mineral density (BMD) and body composition, but measurements vary among instruments from different manufacturers. We sought to develop cross-calibration equations for whole-body bone density and composition derived using GE Healthcare Lunar and Hologic DXA systems.This multinational study recruited 199 adult and pediatric participants from a site in the US (n ¼ 40, ages 6 through 16 years) and one in China (n ¼ 159, ages 5 through 81 years). The mean age of the participants was 44.2 years. Each participant was scanned on both GE Healthcare Lunar and Hologic Discovery or Delphi DXA systems on the same day (US) or within 1 week (China) and all scans were centrally analyzed by a single technologist using GE Healthcare Lunar Encore version 14.0 and Hologic Apex version 3.0. Paired t-tests were used to test the results differences between the systems. Multiple regression and Deming regressions were used to derive the crossconversion equations between the GE Healthcare Lunar and Hologic whole-body scans.Bone and soft tissue measures were highly correlated between the GE Healthcare Lunar and Hologic and systems, with r ranging from 0.96 percent fat [PFAT] to 0.98 (BMC). Significant differences were found between the two systems, with average absolute differences for PFAT, BMC, and BMD of 1.4%, 176.8 g and 0.013 g/cm 2 , respectively. After cross-calibration, no significant differences remained between GE Healthcare Lunar measured results and the results converted from Hologic.The equations we derived reduce differences between BMD and body composition as determined by GE Healthcare Lunar and Hologic systems and will facilitate combining study results in clinical or epidemiological studies. ß

Objective: To investigate the influence of age, sex, ethnicity and total fatness on central obesity in four ethnic populations. Design: Cross-sectional analysis of study subjects enrolled from 1993 to 2005. Subjects: A multi-ethnic (Caucasian (CA), African-American (AA), Hispanic-American (HA) and Asian (As)) convenience sample of 604 men and 1192 women (aged 18-96 years, body mass index 15.93-45.80 kg/m 2 ). Measurements: Total body fat (TBF) and truncal fat were measured by dual-energy X-ray absorptiometry. General linear regression models were used to test for independent associations with log 10 -transformed truncal fat. Results: For all ethnicities, men had a lower percent body fat and more truncal fat than women. Log 10-transformed truncal fat increased with TBF approximately as a square root function. At older ages, there was a greater amount of truncal fat in CA, HA and As men (B0.20-0.25 kg/decade) with the effect more pronounced in AA men (B0.33 kg/decade). For women, the increment of truncal fat per decade was reduced in CA and AA women (B0.07 kg) compared with As and HA women (B0.33 kg). Adjusted for mean values of covariates in our sample, AA had less truncal fat than As. Conclusion: The accumulation of truncal fat is strongly related to age, ethnicity and total fatness in both men and women.

Incidental Findings with Dual-Energy X-Ray Absorptiometry: Spectrum of Possible Diagnoses

New dual-energy X-ray absorptiometry (DXA) technologies provide improved spatial resolution and high image quality. Our purpose was to review DXA examinations to detect collateral findings and to understand their potential impact on patient healthcare. We retrospectively and randomly reviewed 739 DXA examinations (191 of 739, 25.8 %, whole body; 96 of 739, 13.0 %, vertebral fracture assessment; 231 of 739, 31.3 %, lumbar spine; 221 of 739, 29.9 %, femur) that were performed in our institution with a new DXA equipment. Whenever an extra finding was discovered, the physician's report was read and the clinical history of the patient was investigated to understand whether that finding was already known, as well as to check the diagnosis. The population included 208 male and 531 female subjects (58 ± 14 years old). Incidental findings were detected in 117 (15.8 %) of 739 DXA examinations (17 of 117, 14.5 %, whole body; 41 of 117, 35.0 %, vertebral fracture assessment; 32 of 117, 27.4 %, lumbar spine; 27 of 117, 23.1 %, femur): biliary and urinary stones (4.8 %), vascular calcifications (33.7 %), other soft tissue calcifications (25.3 %-e.g., tendons, lymph nodes, intraparenchymatous calcifications), vertebral abnormalities (14.5 %), other bone abnormalities (12.1 %), and morphovolumetric alterations or abnormal anatomical structures (9.6 %). Among all these findings, 50 (42.7 %) of 117 could be verified by other imaging modalities. Forty-nine (98.0 %) of 50 incidental findings were identified as true findings, and DXA was able to orient the diagnosis (exact diagnosis in 37 of 50, 74.0 %); however, none of them was mentioned on available DXA reports. An interpreting physician should treat the DXA image with the same attention given to any other X-ray image. Sometimes DXA may allow a qualitative diagnosis of collateral findings. However, potential negative effects on healthcare economy should be considered for false-positive or insignificant findings.
