Objective: To compare the effectiveness of two drugs, polyethylene glycol 4000 without electrolytes and magnesium hydroxide, in the treatment of chronic functional constipation in children.Methods: Thirty-eight children were randomly assigned to either of two groups, polyethylene glycol 4000 without electrolytes or magnesium hydroxide. The children were followed through periodic appointments until they reached 6 months of treatment. In each medical appointment the following aspects were evaluated: stool consistency, frequency of bowel movements, fecal incontinence, abdominal pain, straining and acceptance of the drugs.Results: Seventeen children made use of polyethylene glycol and twenty-one received magnesium hydroxide. All variables analyzed improved for both groups, with no statistically significant differences. All children accepted polyethylene glycol, while 42.9% refused magnesium hydroxide. Conclusion:The two laxatives showed no difference in effectiveness for the treatment of constipation. However, due to its better acceptance, because it is odorless and tasteless, polyethylene glycol proved to be a better option for treating chronic functional constipation.J Pediatr (Rio J). 2011;87(1):24-28: Treatment, children, constipation. ResumoObjetivo: Comparar a efetividade de dois medicamentos: hidróxido de magnésio e polietilenoglicol 4000 sem eletrólitos no tratamento da constipação intestinal crônica funcional em crianças. Métodos:Trinta e oito crianças foram divididas, por processo randô-mico, em dois grupos para uso de polietilenoglicol 4000 sem eletrólitos ou de hidróxido de magnésio. As crianças foram acompanhadas em consultas periódicas até completarem 6 meses de tratamento. Em todas as consultas, foram investigados: consistência das fezes, frequência evacuatória, presença de escape fecal, dor abdominal, esforço evacuatório e aceitação desses fármacos.Resultados: Dezessete crianças fizeram uso de polietilenoglicol, e 21 utilizaram o hidróxido de magnésio. Houve melhora clínica de todas as variáveis nos dois grupos, sem diferenças com significância estatís-tica. Todas as crianças aceitaram o polietilenoglicol, enquanto 42,9% recusaram o hidróxido de magnésio.Conclusão: Não houve diferença de efetividade no tratamento da constipação intestinal entre esses dois laxantes. Entretanto, a melhor aceitação do polietilenoglicol, por ser inodoro e insípido, torna-o melhor opção no tratamento da constipação intestinal crônica funcional.

Background: Constipation is one of the most common problems in the pediatric age group. In the vast majority, chronic constipation is idiopathic or functional. In addition to education and behavioral changes, disimpaction and maintenance treatment with medications are the main pillars of successful outcome. The medications are variably effective and have potential side effects. Objectives: The current clinical trial was designed to evaluate clinical efficacy of polyethylene glycol (PEG) 3350 and liquid paraffin in chronic functional constipation. Besides, subjects were followed carefully for the side effects. Material and Methods: This study was involved 110 children suffering from chronic functional constipation. They were placed into two different groups, randomly. Each group was treated with PEG 3350 or liquid paraffin. The clinical efficacy and side effects were monitored. Results: After disimpaction with bisacodyl suppositories or combined with Mineral oil enemas in hard fecal impaction, maintenance therapy with PEG 3350 or liquid paraffin was started. The treatment duration and efficacies were not different between the two groups. Furthermore, serious adverse drug reaction was not observed. The family history of constipation and presence of incontinence were two worse prognostic factors. Conclusion: No difference was observed between PEG 3350 and liquid paraffin in chronic functional constipation in children in terms of their efficacies. They are safe and effective medications in this age group.

Нарушения стула у ребенка являются частой причи-ной обращения к врачу-педиатру. По данным нашей кли-ники, на специализированном гастроэнтерологическом приеме дети с запором составляют ежегодно около 12-14% от всех обратившихся.Редкое опорожнение кишки может отмечаться в любом возрасте, начиная с первых месяцев жизни ребенка, однако более типична эта проблема для детей в периоды введения твердой пищи, приучения к туалету и в начале посещения детских организован-ных коллективов. Выраженность и продолжительность симптомов запора вариабельны: у многих детей нор-мальный стул восстанавливается без особых усилий, у других ситуация может осложняться болезненностью при дефекации, появлением крови в кале и его недер-жанием. В этом случае задача врача состоит в органи-зации своевременной и адекватной помощи малышу, поскольку длительное течение этой формы патологии существенно затрудняет ее лечение. В большинстве случаев проблема запора является непродолжитель-ной, и дети старше 3-4-летнего возраста обращаются по поводу запора реже. По нашим данным, 23% обра-щений приходится на детей в возрасте до 3 лет. Однако, как показывает опыт, редкие обращения не означают редкости проблемы, поскольку взрослеющий ребенок может не сообщать об этой деликатной проблеме даже родителям, страдать психологически и физиче-ски в одиночку и длительное время опорожнять кишку
