Pediatric Traumatic Intracranial Aneurysms

Abstract: During the past 17 years, 7 children have been treated at Children’s Hospital Los Angeles (CHLA) for traumatic intracranial aneurysms. These patients presented with subarachnoid or intraparenchymal hemorrhage (5 cases), growing fracture (1 case), and behavioral change (1 case). All patients were managed with surgery, and no patient died or was made neurologically worse as a result of such management. The existence of a traumatic aneurysm should be considered when new neurological symptoms develop in a patient … Show more

“…1,10,17,26,28) Pediatric and adult aneurysms show several differences, but the evaluation and management is similar. We present a case of massive posterior inferior cerebellar artery (PICA) aneurysm in an infant manifesting as symptoms of a mass lesion, marked hydrocephalus, and aneurysm rupture following a shunt operation.…”

“…Os aneurismas na população infantil são usualmente sintomáticos 3 , destacando-se os sinais de hemorragia subaracnóide, presente em 70 a 80% dos casos em que ocorre aneurisma gigante, como no caso por nós relatado 5,14,18 . A sintomatologia também pode decorrer do efeito de massa focal com déficit neurológico, seja pela compressão ocasionada pelo próprio aneurisma gigante, seja pela formação de hematoma intracraniano.…”

“…Trauma is a very important cause of intracranial aneurysm in this patient group. 8,12,73,77 Although traumatic aneurysms represent only approximately 1% of all such lesions, they can comprise up to 50 to 75% of pediatric aneurysms in some series 37 (this probably reflects the rarity of de novo aneurysms in children). Unusual cases have been presented following nonaccidental injury or birth trauma.…”

“…57 The largest pediatric series was recorded in Los Angeles by Yazbak and colleagues. 77 Seven children were treated between 1975 and 1992, and all underwent surgical repair of their aneurysm. No patient died or worsened neurologically.…”
