This chapter provides an overview of pediatric palliative care in the community setting, where care can be complex and multilayered. The chapter describes the role of the pediatric palliative advanced practice registered nurse (PP APRN) in this setting. This includes the transition of care for a child with serious illness from hospital discharge, to care at home, to managing a home death. At times, community palliative care can be intimidating for families, caregivers, and professionals. However, with education and support, families are empowered as experts about their children and can provide the care their children need in a community setting. The PP APRN, along with the team, guides and helps the family care for their child in the place they feel most comfortable.
