Pediatric Brucellosis: A Retrospective Evaluation of 82 Cases

Abstract

Amaç: Güneydoğu Anadolu bölgesi ülkemizde brusellozun endemik görüldüğü coğrafyada bulunmaktadır. Çalışmamızda bruselloz olgularının epidemiyolojik, klinik, laboratuvar bulguları ve tedavi yanıtlarını değerlendirilmesi amaçlandı. Gereç ve Yöntemler: Ocak 2009-Eylül 2011 tarihleri arasında bruselloz tanısıyla izlediğimiz 82 olgu yaş, cinsiyet, geliş yakınmaları, bulaş yönünden riskleri, aile öyküsü, fizik muayene ve laboratuvar bulguları, uygulanan tedaviler açısından geriye dönük olarak değerlendirmeye alındı. Tanı yüksek brusella aglütinasyon titresi ve/veya kültürde etkenin üretilmesi ile konuldu. Bulgular: Olguların 37'si erkek 45'i kız olup, yaş ortalamaları 11.96±3.57 (yaş aralığı: 5-16) yıl idi. Olguların çoğunu (%76.8) kırsal kesimde yaşayan, tarım ve hayvancılıkla uğraşan ailelerin çocukları oluşturmaktaydı. En sık başvuru nedenleri ateş (%86.6), eklem ağrıları (%75.6), halsizlik (%51.2) ve terleme (%37.8) idi. Fizik muayenede; ateş (%90.2), artrit (%26.8), splenomegali (%22), hepatomegal (%20.7) ve lenfadenopati (%14.6) saptandı. Brusella aglütinasyon titresi tüm olgularda 1/160 ve üzerindeydi. Bir olgunun kan kültüründe, bir olguda beyin omurilik sıvısında üreme oldu. Sekiz yaş ve üzerinde olan 68 hastaya doksisiklin ve rifampisin, sekiz yaş altında olan 14 hastaya trimetoprim-sülfometaksazol (TMP/SMX) ve rifampisin kombinasyon tedavisi uygulanmıştı. Sonuç: Bruselloz çok değişik klinik belirtilerle karşımıza çıkabilmektedir. Hastalığın endemik olduğu bölgelerde özellikle ateş ve eklem ağrısı yakınması olan olgularda öncelikle düşünülmesi gereklidir.
