Pedal gangrene in a patient with COVID-19 treated with prone positioning and extracorporeal membrane oxygenation

Abstract: Many patients hospitalized with coronavirus disease 2019 are treated with venovenous extracorporeal membrane oxygenation and prone positioning to optimize oxygenation. However, this combination can result in lower extremity tissue necrosis, especially without adequate offloading. We report the case of a 31-year-old man who required mechanical ventilation and venovenous extracorporeal membrane oxygenation secondary to complications from coronavirus disease 2019, and subsequently developed pedal dry gangrene. Th… Show more

“…Furthermore, pedal gangrene associated with VTE in the context of COVID-19 infection has been reported in some case reports. 8 , 9 , 10 , 11 , 12 Currently, methods such as anticoagulation alone, mechanical thrombectomy, catheter-directed thrombolysis, prone positioning with veno-venous extracorporeal membrane oxygenation, and amputation have been trialed with varying levels of success. 8 , 9 , 10 , 11 , 12 In other similar reports, COVID-positive patients with concomitant VTE and arterial thromboembolism (ATE) ultimately required more proximal amputation in the acute setting.…”

“…In this case, no angiogram was performed to identify a thrombus, furthermore, the gangrene was not progressive. He had recovered with no loss of life and limb [ 14 ]. However, in both our cases, an angiogram revealed thrombosis of common femoral and external iliac in case one, thrombosis below the popliteal trifurcation in case two.…”
