PEACE: Perception and Expectations toward Artificial Intelligence in Capsule Endoscopy

Abstract: Artificial intelligence (AI) has shown promising results in digestive endoscopy, especially in capsule endoscopy (CE). However, some physicians still have some difficulties and fear the advent of this technology. We aimed to evaluate the perceptions and current sentiments toward the use of AI in CE. An online survey questionnaire was sent to an audience of gastroenterologists. In addition, several European national leaders of the International CApsule endoscopy REsearch (I CARE) Group were asked to disseminate… Show more

“…Forty included studies mentioned potentially replacing physicians and changes in employment market caused by clinical AI. The support rate for the statement that AI could replace human physicians ranged from 6 to 78% ( 19 , 37 , 58 ), of which 31 (77.50%) studies showed that the support rate was less than half ( 11 , 16 19 , 21 , 22 , 24 , 25 , 30 , 33 37 , 39 42 , 44 50 , 55 , 56 , 59 , 60 , 63 ). Radiologists did not view AI as a threat to their professional roles or their autonomy, however, radiographers showed greater concern about AI undermining their job security ( 68 ).…”

“…This particular need is going to become even more important in the next few years, as the use of artificial intelligence (AI) software for endoscopy diagnosis is expected to markedly increase. Large CE databases with solid and shared findings between experts are keystones for effective machine learning and for building precise and trustworthy AI programs [ 24 , 25 ]. The results of our work have shown that the main features of atrophic lesions are mosaicism, scalloping, and folds reduction, and they have rapidly found consensus among readers of SBCEs, even among those who read only a few CEs of coeliac patients per year.…”

“…Finally, ethical issues related to physician’s acceptance and patient’s confidence in novel computer-aided algorithms such as onboard AI needs to be discussed. Even though studies such as PEACE 37 clearly indicate that there is a significant tendency towards AI acting as adversarial rather than assistive tool, (i.e., only selected images will be proofread by the expert, while others will simply not be double-checked), the fact that not all the images are available for a full screening process could be a cause for concern. Our platform, using OTAP, is capable of transmitting all images, if pre-indications on various lesions exist.…”
