Pauci-Immune Necrotizing and Crescentic Glomerulonephritis with Membranous Lupus Nephritis, Fifteen Years after Initial Diagnosis of Secondary Membranous Nephropathy

Abstract: Renal involvement in systemic lupus erythematosus (SLE) is usually immune complex mediated and may have multiple different presentations. Pauci-immune necrotizing and crescentic glomerulonephritis (NCGN) refers to extensive glomerular inflammation with few or no immune deposits that may result in rapid decline in renal function. We report a case of a 79-year-old Hispanic male with a history of secondary membranous nephropathy (diagnosed by renal biopsy 15 years previously) who was admitted with acute kidney in… Show more

“…In addition to these cases of overlap syndrome reported in the literature, there have also been cases of ANCA positive glomerulonephritis in the setting of SLE as well as ANCA negative glomerulonephritis outside the setting of SLE [1,4,7,[10][11][12]14]. There have also been cases of pauci-immune glomerulonephritis reported in the setting of concurrent lupus nephritis [4,8,15].…”

“…In a significant number of patients, ANCAs are also detected [105]. Biopsy examination mainly reveals focal segmental glomerulosclerosis or mesangial proliferative glomerulonephritis lesions [106].…”

“…Biopsy examination mainly reveals focal segmental glomerulosclerosis or mesangial proliferative glomerulonephritis lesions [106]. In certain cases, ANCAs may be associated with membranous lupus nephritis [105]. ANCA-associated necrotizing and crescentic glomerulonephritis may present as superimposed nephropathy on lupus nephritis [107].…”
