Objective To determine the relationships between American style football (ASF) participation, acquired left ventricular (LV) hypertrophy, and LV systolic function as assessed using contemporary echocardiographic parameters. Background Participation in ASF has been associated with the development of hypertension and LV hypertrophy. To what degree these processes impact LV function is unknown. Methods This is a prospective, longitudinal, cohort study evaluating NCAA Division I collegiate football athletes stratified by field position (linemen, n=30 vs. non-linemen, n=57) before and after a single competitive season with transthoracic echocardiography. LV systolic function was measured using complementary parameters of global longitudinal strain (GLS, 2D speckle-tracking) and ejection fraction (EF, 2D biplane). Results ASF participation was associated with field position-specific increases in systolic blood pressure (linemen Δ SBP = 10±8 mmHg vs. non-linemen Δ SBP = 3±7 mmHg, p<0.001) and an overall increase in incident LV hypertrophy (pre = 8% vs. post = 25%, p<0.05). Linemen who developed LV hypertrophy had concentric geometry (9/11, 82%) with decreased GLS (Δ = −1.1%, p<0.001) while non-linemen demonstrated eccentric LV hypertrophy (8/10, 80%) with increased GLS (Δ = +1.4%, p<0.001). In contrast, LV ejection fraction in the total cohort and when stratified by field position was not significantly affected by ASF participation. Among the total cohort, lineman field position, postseason weight, systolic blood pressure, average LV wall thickness, and relative wall thickness were all independent predictors of postseason GLS. Conclusions ASF participation at a lineman field position may lead to a form of sport-related myocardial remodeling that is pathologic rather than adaptive. Future study will be required to determine if targeted efforts to control blood pressure, minimize weight gain, and to include an element of aerobic conditioning in this subset of athletes may attenuate this process and translate into tangible downstream health benefits.

Elder abuse and associated factors in eastern romania

Background This article explores elder abuse in a hospitalised population. We wanted to identify details related to psychological and emotional abuse in the older population in our region and to determine the importance of the Elderly Abuse Suspicion Index (EASI©) in comprehensive geriatric assessments. Methods This cross‐sectional study conducted between March 2015 and May 2016 included 386 consecutive hospitalised patients over 65 years of age. All patients underwent a geriatric assessment, data were collected about their medical history, and the EASI© was administered to each. The main outcome was identifying the presence, the type of abuse and the factors associated with abuse. Results There were 21.5% of patients who suffered any form of abuse. Women were more frequently abused than men. Emotional abuse was the most common (60.2%) followed by neglect (53%) and physical abuse (22.91%); sexual abuse was absent in our study group. The abused patients had an impaired cognitive function (P = 0.034). They were also malnourished (P ≤ 0.001) and depressed (P = 0.001). The presence of peripheral artery disease, stroke, pneumonia, chronic kidney disease, musculoskeletal diseases and anxiety correlated with the presence of abuse. No statistically significant correlation was found between the degree of independence in instrumental activities of daily living and the presence of abuse (r = 0.105, P = 0.051). Conclusions EASI is a tool for detecting elder abuse and should be included in the standard geriatric assessment to prevent ageism. The number of abused elderly patients is significant, and the multiple factors associated with abuse are diverse.

Федеральное государственное бюджетное образовательное учреждение высшего образования «Читинская государственная медицинская академия» Министерства здравоохранения Российской Федерации, Чита, Россия Цель Изучение закономерностей формирования структурно-функциональных нарушений левого желудочка (ЛЖ) у мужчин с подагрой. Материалы и методы В исследование включено 114 мужчин с подагрой с интермиттирующим (n = 80, средний возраст 46,34±6,14 года) и хроническим течением (n = 34, средний возраст 49,03±5,66 года). Всем пациентам проведено суточное мониторирование артериального давления (СМАД), эхокардиографическое исследование (ЭхоКГ) с идентификацией геометрической модели архитектоники ЛЖ и оценкой диастолической функции методом допплеровской ЭхоКГ. Результаты Основную группу исследования составили 98 пациентов (85,9%) c наличием артериальной гипертензии (АГ), группу сравнения-16 (14,1%) больных с отсутствием повышения артериального давления (АД). У пациентов с подагрой в 67,6% случаев регистрировалась диастолическая дисфункция ЛЖ, частота которой увеличивалась у пациентов с хроническим течением и наличием АГ. У 52% мужчин с подагрой, ассоциированной с АГ, была выявлена концентрическая гипертрофия ЛЖ. Пациенты с избыточным ночным снижением АД (гипердипперы) значительно чаще встречались среди нормотоников (39,2% против 13,8%), а больные с отсутствием ночного снижения и ночным повышением АД (нондипперы и найтпиккеры, 41,6% против 18,9%)среди страдающих АГ. Установлены положительные корреляционные связи мочевой кислоты сыворотки крови с индексом массы миокарда ЛЖ
