Patterns of Filler-Induced Facial Skin Ischemia: A Systematic Review of 243 Cases and Introduction of the FOEM Scoring System and Grading Scale

Abstract: Background: The incidence of facial skin necrosis has increased considerably because of the growth in the popularity of dermal fillers. This study describes the patterns and severity of facial skin ischemia, along with associated neuro-ophthalmologic injuries, in the published literature through the introduction of the facial artery, ophthalmic artery, distal external carotid artery, internal maxillary artery (FOEM) facial angiosome scoring system and grading scale. Methods: A systematic review of all photog… Show more

“…2). 5 The current case report describes the first published instance of hemi-palate necrosis arising from accidental intravascular injection of filler into the cheek. This injury, classified as F 0 O 1 E 0 M 2 (Grade 3A), likely ensued following accidental cannulation of the infraorbital artery, a branch of the distal internal maxillary artery (IMax), resulting in retrograde arterial occlusion of the ipsilateral descending palatine artery.…”

“…2). 5 This is likely due to the more superficial course of the angular artery and the high frequency of treatments targeting that region of the face.…”

“…Filler-inducedvascularinjuriesareapotentiallydevastatingiatrogenic etiology in aesthetic medicine, with HA fillers comprising roughly80%ofcases. 3 Althoughthemechanismofinjuryunderlying these rare events is incompletely understood, accidental intravascular inoculation resulting in occlusion remains one of the leading theories. 4 Consequently, knowledgeoftheintraluminalbehaviorof 9 In nature, arborization of arterial networks obeys Murray's law, which means that, as the vessel diameter decreases witheachbranchgeneration, thereisasubstantialincreaseintotal cross-sectionalarea(Figure 3).…”
