Patterns of Failure Following CT-Based 3-D Irradiation for Malignant Glioma

Abstract: Limited volume irradiation of malignant glioma with a target volume 3 cm beyond that indicated by preoperative CT scan seems to be sufficient, since 93.5% of recurrences were completely within 3 cm distance from the margin of the tumor. Improvement of the results with larger treatment volumes are unlikely. Complete irradiation of perifocal edema was not found to be necessary.

“…Image quality and window level [41] can also result in over-and underestimation of tumor boundaries and thereby increase inconsistency of target volume definition among observers. Selection of adequate imaging techniques, good quality of all images used for treatment planning, and exploitation of all available technical means for image registration and multimodality imaging [1,23,30,32] are vital for increased accuracy in target volume definition. The intraoperative positioning of markers in the tumor bed appears helpful.…”

“…With standard treatment consisting of surgery and postoperative radiotherapy, the prognosis of patients with malignant glioma is poor [1,12,16,18,27]. The application of additional chemotherapy and different fractionation schedules did not improve the outcome of these patients [5,8,17,26,29,30].…”

“…Neurologische Defizite bei dem Strahlentherapeuten vorgestellten Patienten werden zumeist durch hirneigene Tumoren, Hirnmetastasen oder eine metastatisch bedingte Rücken-markkompression verursacht [2,18,20,27,34,50]. Als Ursache kommen jedoch auch eine Meningeosis carcinomatosa [19] oder benigne Tumoren in Frage.…”
