Background and Aims: Acute rheumatic fever and rheumatic heart disease constitute an important public health problem in the developing countries. The disease results from an abnormal autoimmune response to a group A streptococcal infection in a susceptible host. We aim to describe the pattern of valvular involvement in patients taking Injection Benzathine Penicillin.Methods: All the patients, who were taking injection Benzathine penicillin during 15th April to 14th July, 2013 at our Centre, were included in this study. Demographic features like age, sex, echocardiographic diagnosis along with any adverse effects of Benzathine penicillin were collected.Results: A total of 661 patients were included in our study, out of which female predominated in numbers. Rheumatic heart disease rather than rheumatic fever was the cause for Penicillin injection. Mitral valve was the most common valve involved and it was more common in female.Pure mitral stenosis was the most common valvular involvement. Forty two percent patients underwent intervention; among them Percutaneous transluminal mitral commisuorotomy was the most common. There was no adverse event during the study time following penicillin injection.Conclusions: Women are more commonly affected than male. Mitral valve is the most common valve involved. BACKGROUNDRheumatic heart disease (RHD) is a major contributor to morbidity and premature death in poor and developing countries like India. RHD predominantly affects young and working age group. Objective-To evaluate relative frequency of the various cardiac valvular lesions in Rheumatic heart disease (RHD) in patients who underwent Echocardiogram in the Department of Cardiology, Thanjavur Medical College, Thanjavur, Tamilnadu. MATERIALS AND METHODSThis retrospective descriptive study was conducted at Cardiology Department based on transthoracic two dimensional echocardiogram reports, diagnosed as RHD (Rheumatic heart disease) from January 2016 to December 2016. The reports were retrieved from computerised database and demographic features like age, sex, type of valvular lesion were analysed. RESULTSOut of 17,625 patients who underwent echocardiogram in the Department of Cardiology, 428 patients (2.4 %) were diagnosed with rheumatic heart disease. The commonly affected age group was 21 to 40 years. The distribution being 120 males (28%), 288 females (67.2%) and 20 children (4.6%). Mitral Regurgitation was the predominant valvular lesion affecting 328 (76.6%) patients with female preponderance of 239 (72.8%) patients. The second most common valvular lesion was Mitral Stenosis. It was diagnosed in 287 patients constituting 67.05%. The incidence was most common in females (205 cases, 71%) when compared to males (82 cases 29%). Of those 287 patients, 139 (48.4%) patients had severe MS and 92 (32.0%) had moderate MS. Aortic Regurgitation (AR) was present in 126 (29.4%) patients and almost always (99 %) in combination. Mixed valvular lesions (MS with MR) were present in 117 (27.3%) of patients. The least common lesion was Aortic stenosis (AS) noted in 24 (5.6%) patients. Out of 118 patients with multivalvular lesions, MS with MR with AR were found in 65 (55.08%) patients. CONCLUSIONThe most common valvular lesion reported in our study was multivalvular lesion. The predominant pure lesion was Mitral regurgitation followed by Mitral stenosis, Aortic regurgitation and MS with MR.

Background and Aim : Echocardiograph is an important diagnostic tool to evaluate cardiac disease and is indispensible for management. So it is important to know the spectrum of cardiac abnormalities that can be detected by echocardiograph and the frequency of these findings may vary depending on where the echocardiogram is performed. Aim of this study was to find out the spectrum of echocardiographic finding in different age group in a medical college hospital. Methods and materials : A retrospective observational study was done to at Dhulikhel Hospital to review Echocardiographic profile of 3310 patients who were indicated for echocardiogram over a period of 3 years. Data collected from echocardiograph report registry. Data analysis was done using SPSS 17. Result : Congenital Heart Disease(CHD) (37.74%),Normal finding( 21.19%), Pericardial Heart Disease(19.21%) and Rheumatic Heart Disease(RHD)(17.88%) were the echocardiographic finding in children. In adolescents and young adults Rheumatic Heart Disease (49.90%), Hypertensive Heart Disease (13.34%), Congenital Heart Disease (6.58%), and Pericardial Disease (4.38%) were found. Most common finding in middle age was cor-pulmonale (34.76%) followed by Diastolic Dysfunction (20.60%), Hypertensive Heart Disease(17.06%),Ischemic Heart Disease(IHD) (12.80%). In elderly age most common finding was Diastolic Dysfunction (57.14%). Conclusion : The spectrum of echocardiograph finding in a medical college hospital ranges from Congenital Heart Disease, Rheumatic Heart Disease, Ischemic Heart Disease ,Pericardial disease, Corpulmonale, Diastolic Dysfunction, systolic dysfunction and degenerative valve disease. Streptococcal sore throat leading to Rheumatic Heart Disease and complication of tuberculosis and parasitic infestation leading to pericardial diseases, as well as Hypertensive Heart Disease, and Ischemic Heart Disease constitutes burden in Nepal.
